Do it politely, but do it consistently. Just put in the flag, "Doctor speaking too quickly to comprehend." After a while (a loooooong while, you know how they are), they'll get the message. If they don't, then that means that they don't mind blanks. Either way, you save your sanity and your pocketbook.
