what I've seen when in my personal doctor's office. It is a program (I'm sure there are many different programs available) on a computer that stores information about you, the patient. When you visit your doctor, the information is updated. The doctor, in my case, asked me questions and sat behind his computer and typed away, essentially eliminating his need to dictate or write in the chart.
