Only we do not have QA.  We send work directly to the clinic.  The physicians are always complaining if we do not spell a physician's name correctly.  They dictate, "The patient has seen Dr. _____ in the past."  No indication where this doctor is.  I do not mind doing a quick research to try to find this physician.  However, if you do not know how to spell his/her name, what city he/she is in, or sometimes even what state he/she is in, you could spend half a day and not find it.  Lately, if I cannot find it with a reasonable search, I leave a blank to flag it.  Also, a lot of doctors really stray from their submitted format.  I would hate to be the QA person that had to deal with this, but as an MT, sometimes I just do not know what to do with this myself.
