Serving Over 20,000 US Medical Transcriptionists

I have worked as a QA and as an MT which makes me an expert at SM

absolutely nothing, but I'm going to give my opinion anyway.

Here's the problem with the whole QA process.  We (MTs) are told when interviewing that this or that service strictly adheres to the great and almighty, BIBLE of medical transcirption, the BS, er I mean BOS.  Then, when hired and given our assigned accounts, we are told the BOS applies, but .....  we do it this way for this client, that for that client, and the other way for another client.

So, we as MTs are supposed to sift through all the crap and put out a flawless document and on top of all that, we now must also consider the egomaniacal QA person who has their "own style" of QA?

So, now we must take an awful dictator who can scarcely put together a coherent sentence and make sense of what they are saying, then we have to make sure that whatever sentence we've constructed to articulate the dictator's thoughts strictly adheres to the rules of the BOS but WAIT then we must make sure that we've broken the correct BOS rules to suit the account specifics, and then we must now contend with the QA person who has decided we should dot our I's with little hearts cause that's the way she likes it?

And we do all of this for 8 cpl ?!?!?!?!?!

Somebody stop this crazy merry-go-round, I feel sick!

I have worked for OSI for a while but this makes me want to look elsewhere,
nm
Two of the best companies I've worked for didn't make me test. Makes you wonder if...sm
these MTSOs that mandate so many "tests" aren't just trying to see how many actual reports they can get transcribed for free. I could understand maybe a written test and one very in-depth transcription test, but 6 is utterly ridiculous. I would have definitely passed on that too and then told the MTSO exactly why.
Not an expert, but...

Hello

I am also in the process of going digital.  Some things I have learned along the way.....

1. I bought the Olympus DSS 330 for each of my docs.  It's reasonably priced, easy to use, and software works great.  I bought the recorders, because the doctors were completely happy with the tapes, and I am the one who wants to cut down on travel time and gas, so I felt like I should supply the recorders.

2. Audio files are too big (at least for my e-mail) to transport via regular e-mail, and they have to be secure or encrypted, so I bought space (personal/small business account) on CryptoHeaven for 2.49/month.  This is supposed to be HIPAA compliant and secure.  The files get uploaded to this site and can be transferred back and forth between you and the doctors' offices.  Keep in mind, if you or the docs have dial up, this process takes forever, so it is good to know what type of internet connection your docs have.  Cable and DSL are much faster.  If they have dial up, it could take up to 30 minutes for them to upload one day of dictation or even more.  Just a side note... I have found that audio files in DSS format are the smallest and thus the fastest to transfer.

3. I also use WinZip with this as an added protection.  The practice felt comfortable with the protection of WinZip, but WinZip alone does not provide the vehicle to transport the large DSS audio files, and DSS audio files cannot get compressed much more, so that is why I went with CryptoHeaven.

You can e-mail me if you would like.  Keep in mind, I am no expert, and I am finding out things as I go along.  I will let you know how it goes.

I'm no expert, but...

Do you have a "sample" report of something you've already done in whatever format you're currently working in?  If you do, you could take your report and figure out how much you made doing that report in the format you're currently working in.  Then copy the report and put it in a new document with Times New Roman 11 and one-inch margins.  Then, using 0.9 gross lines, figure out how much you would make with the new format.  Compare it to the old.  Maybe do this with a few different reports to get an idea of a true comparison.

Hope this makes sense. :)

Not an expert here, but
I have had the same trouble, as well as having the longer files only play about a minute or two of an 8+ minute dictation (but does not go back to the beginning). I use the Olympus software, and have the trouble when I am playing .wav files rather than .dss. Two things that I have done are:

1. Download the Express Scribe free transcription module. (The trouble I have had with this is that my footpedal will not work with Express Scribe, but it works with hot keys also, so in a pinch this is what I've done.)

2. Downloaded a free trial of a program called GoldWave (I am sure there are probably others available.) This allows you to convert the file type (I convert to a .wma format -- Windows Media Audio, I think.) My Olympus plays the .wma files in full, but it seems to lose a little clarity on conversion.
Well, I am not an expert (sm)
in this area (not even close), but I would guess that anyone who installs Spyware on your computer can monitor your activity whether you are connected to a VPN or not.  What exactly is spyware, how would it get onto your computer, who put it there, why would they put it there are all questions I cannot answer.  When I was having security issues with my computer, I went to a great site (www.techsupportforum.com).  They have a 'General Computer Security' section that explains what you need to protect yourself in general.  You can also ask questions there and they will help you figure out if anything is already installed on your computer.
I'm no expert, but
are the headphones plugged into the speaker, or where?
Need some expert guessers, please sm.

Even my QA person can't get this one, so I thought maybe someone here might have an idea.  Podiatric.......the doc says, "The patient relates that approximately four months ago, the patient stepped into a hole and twisted his foot.  He heard a pop and has since had intermittent heel pain.  The patient does relate ______  dyskinesia with pain on extended periods of weightbearing."

Once, it sounded more like prosthetic, only without the "r" sound, and the second time maybe even like postatic, which I know is not a word.  Any help would be much appreciated as he has gotten me on this twice already.

Were it my personal account, I'd just call him and ask him!

consulting expert

does anyone know of where I can find or how I can go about looking for a consulting person to help me out with trying to get local accounts and what I need and should do such as contract, how to go about charging the client, what software is needed and the ins and outs of various ways of getting the work back to the client?  any information would be so greatly appreciated.  I just need pointed in the right direction.

Or can somone give me advice as to what they did to get up and running?

Have a beautiful day!

I'm not an expert in pathology

but it seems the experience that I have had with these, they are fairly short.  I guess they could be lengthy, but I would say on an average $3.00 a page should make you a mighty fine living.  You may want to see some samples before pricing this one out.  The experiences I've had were with a hospital, and these were fairly short reports.  The pathology department employed two full-time hourly MTs for a hospital that probably held about 650 beds.  The OR had about 12 rooms.  The ambulatory surgery department had about 5 rooms.  These are all just my opinions though.  Good luck to you!

Need fax machine expert!
Can anyone tell me if you have ever had the problem of your fax machine not being able to fax only to one particular number?  I can fax anywhere else, but to this one particular number.  I have tried everything I know, even so far as calling the maker of my fax machine, but no one can tell me why I get a communication error when faxing to this one particular number.  I have changed my splitter on that line, my filter on that line and had the phone company check out my line, all to no avail. It will go through every now and then, but mostly it will not go through.  This has just started happening in the last few months. If anyone has every had this problem and can give me advice, I'd appreciate it immensely!  Thanks in Advance!
I'm no SH expert but you can make
an entry so that it expands A1c.
I'm not even CLOSE to being an expert but..Have you tried this..

Going into the printer properties and checking your printing preferences. If your not sure how to do this....Go to your Control Panel, open up Printers...find the printer, right click on it, hit properties, and under the General tab, there should be an area at the bottom titled "Printer/printing Preferences (something like that)..hit that and check out all your settings. I'd play around in that areas with your settings to see if somehow something is in there wacko.. Just a thought. I hope it works..

EXPERT VISTA HELP, PLEASE!

I just downloaded the EditScript (Escription) program onto my computer but I'm not getting any prompts from the Installation Wizard to install it.  This is a brand-new computer and I'm kind of a Vista imbecile since I've never used it.

Is there some other step I need to take to get the program to actually install onto my computer so I can use it, or have I done something wrong?

I'm SOOOO confused and can't reach my supe to ask her, so I was hoping someone here would be able to offer some advice.

Thanks ever so much!!!

Anyone an expert on Bayscribe software??
I just started a new job that uses Bayscribe, but the audio is so muffled I cannot hear the dications clearly.  The owners have no idea what is wrong, and I called Bayscribe, and they say it is on my end...either sound card or headphones.  I also work for another co. that uses Emdat, and I use express scribe, and the dictations are crystal clear.  Does anyone know what is going on?
Email me. I'm no expert, but I'll tell you what I know. nm

ExText Problems every day. Need some expert advice

HELP! Does anyone else have this problem and what did you do to solve it?

I use ExText and if I do a Cntl-S then Cntl-T, ExText will close both the document I've been viewing AND the job I'm working on, then open a completely new BLANK document, no headers, etc. I still have the voice files, but no document. I lose the job I've been working on as there is no time to save it to disk. It  usually near the end of transcribing a job. It happens every day, usuallly near the end of my shift. My production is suffering because of it, too.

I'm getting so frustrated and don't know what to do anymore. ExText has been reloaded a couple times and that doesn't solve the problem. I've been told it's my Word causing the problem and I have to remove it and reload that program, which I had done months ago, but still have the same problem.

I don't know where else to turn or what to do. Am I destined to NOT work for any company that uses this program? If so, that leaves out a heck of a lot of jobs.

Other than this problem, I really love using ExText. I think it's great...when it works right.

I can image that once we train the VR reports by our expert
editing and corrections, the VR software will start producing more accurate reports. Unfortunately, when that time does arrive, there will not be a need for as many medical editors/transcriptionists as there are now. I suspect at that point we will be competing for MT jobs, and our rates will go even lower than they are now. I hate to paint a somber picture here, but this is what the hospitals have been striving to accomplish since the mid-1980s, when they decided to outsource the transcription to MTSOs to avoid paying hourly rates and benefits.
I just started telling them I was a "medical language expert"
as you truly are one anymore trying to decipher all these docs and for some reason, that title seems to bear more acceptance
YOU may be a typist for a doctor but I'm a MEDICAL LANGUAGE EXPERT!

You can tell I'm not an expert but what is INXS? I mean the songs the bottom three sing are INSX

I have never heard of them and they sound kinda metal to me.  What category does INXS fall into?     I said I know very little about the music that these people are singing for the most part.  Is Nirvana metal?

Thanks.

who died and left her boss and expert on nut-case MTs or freakish behavior?
sounds like one of the MQ supervisors all filled with herself.
Every company is different -- I worked FT for one as an employee but worked a split shift - sm
So I never took breaks. I would work 5 hours, break for about 4 hours then do another 3. Another company I worked for did not care what hours you worked (IC) but wanted a min. amount of work each day, 500 for PT and 1000 for FT-- BUT they paid you by how many lines an hour you put out, the higher the lph the higher up the scale you made per line in pay; they have since changed everyone to a flat rate with incentive. But bottom line, if you are an IC it does not matter what hours you work, though many ask for a schedule and ask you to stick to it, they just want you to meet line requirements daily, i.e. 1000 per day, 1200 per day, whatever it is.
I got up early, worked during naps, and worked when DH got home.

You have to be disciplined to make yourself work when baby is napping instead of maybe watching TV or doing housework, etc.

I might also go the route of having a teen come into your home, or either trying a mother's morning out program at a local church/daycare.   I've been home since my youngest was born and he has never been in all-day daycare, but I did have him in a mother's morning out program 15 hours a week at a local church.   It didn't help a lot with my work schedule because I had an older son in school and was a room mom and tutored other kids, but that might be an option.  The only problem with the mother's morning out program is they are around other kids and tend to pick up every germ.  I finally took my DS out of the program because he stayed sick.  You were supposed to keep them off if they had green nasal discharge and I did, but no one else did.  Every time I got him well after 2 to 3 days back he would be sick again.  Other than that it was very good for him because he would not have had a chance to be around kids his age otherwise.

I worked on that system when I worked there...
The thing that chapped my hide about that TWS was that they designed it so that all the headers of each section of the report were canned and thus you were not paid for them, even though you had to take your hand off the keyboard and mouse down a list of paragraph headers to chose the one you needed.  Just racking up more profits for themselves.  Ticks me off.
that is what is being worked out, and has been worked on for the last few years already...only
why are we just hearing this now? I know voice recog has been around, but this is entirely different. This will also make coding and billing obsolete.

Don't know why, but it just bothers me that one of our 'own' is the one pushing this...and she is also connected with AAMT. Do they support this, and if so, what is their advice I wonder to the MT?

Perhaps that is one question for their website (I do not subscribe to their mag or credentials...)

Thanks, for the input!
Makes me wonder
I've heard how many times that children that are mean to animals are being mistreated in some way. After hearing how this little girl (at only 10 years) is so brutal, makes me wonder how she is being treated by the adults in her life.
I think they do it because it makes you
pay attention to them - you cannot just continue working without at least talking (yelling, touching them to move them) to them.  At this time, they are seeking any type of attention, even if it is negative.   Same with cats flopping out on my keyboard.
That's what makes me so

1/cent for no reason....I'm telling you the CHECK is fulfilling

some legal purpose.  Please run this past an attorney again.

I will try to call Drew, Eckl et all on Monday myself too.

What makes you think
that Zipper. We all LOVE YOU.
You have such a wonderful personality. You bring such warmth and compassion to the board.
makes you wonder...
what happened to the 3 martini lunch?
makes you wonder...
what happened to the 3 martini lunch?
That makes two of us
I want out of MQ DESPERATELY!
Nobody makes that in this biz
nm
that makes 2 of us! sm
Just about a week ago it was 80 degrees and now it is winter all over again. Kinda nice since I don't have to go out tonight though. Just stay warm and dry at the computer.
doc makes me so mad.....
I have a 24-48 hour TAT contracted with 7 docs.  90% of their work is returned in 24.  One of them constantly wants rush jobs, sometimes asking before they've even sent me the work!   Then always had to do him first.  Since each doc's dictation comes at different times, I now have to stop what I'm doing and get his done the minute it comes through.  I just think he's such a demanding jerk!!
She makes that much just as an LPN? What
state is this in, if you don't mind me asking? Thanks!
Yes, it makes since as that is what

You can use Ctrl + Shift + hyphen and Ctrl + Shift + Space for the hard hyphen and hard space.  If your work platform has overwritten these Word settings you can reassign keys for them.  Both commands worked for me in the beginning but IS did some sort of upgrade on my work computer which disabled my hard hyphen (Ctrl + Shift + hyphen).  I tried reassigning it back but no go, so I reassigned Ctrl + H.  Works like a charm plus saves me from hitting the extra key.  I know you can ESP words with hard hyphens.  I don't remember if I tried with the hard space or not (oldtimers!!). I'll have to try tomorrow at work because my interest has been piqued.  For the hyphen, I highlight the words with the hard hyphen, hit Crtl + Ctrl for the ESP box to come up, and then give it a short.  I'm going to try this with the hard space tomorrow to see if it works.

Why is it that you think being an IC makes - sm
you a better transcriber than anyone else on this board? It isn't WHO you work for that makes you worth more than pocket-change, it's how WELL you work.
If she indeed makes anywhere near that, (sm)
it would have to be pulling up multiple-page templates, and typing in the patient's name, over and over and over. Not straight dictation on so many different docs and hospitals that there's no hope of having templates for them.
Okay, when I do that, it makes
the whole thing (cm2) superscript. What am I doing wrong?
Makes you want to
Makes you want to barf in your hand - Ha!

What makes you think we pay OS MTs more?

OS = offshore

Makes me want to cry!

You know, people........ it just makes me want to cry out loud!  I am getting more angry as we speak.. you have all heard from me before about this business, but I'm going to be VERY frank and spill my guts right here in front of you and the whole MT world, hoping that someone who is in the right position will read it and HAVE THE GUTS to make a stance. (Can you say "hospital board of directors?") This is something that AAMT (or whatever the daylights they are now) should be doing for us. It's about time these "high and mighty" doctors listen up!

We are a family of hard-working people who practically have to become doctors just to transcribe these stinking reports.  I don't know about all of you, but I DON'T MAKE A DOCTORS' SALARY! I truly believe that all of you as well as myself try every day like the devil to do the very best we can to participate in patient care and earn a decent living, but as I have said a million times if not more, it is nearly IMPOSSIBLE and at this time, there ARE NO ANSWERS TO ALL THIS CHAOS....

Actually, I take that back... there are a couple answers..We are all reprimanded for the incompetence of the doctors.  THEY ARE NOT GODS as they would like to believe they are. They put on and take off their pants just like we do and when the he _ _ are they going to take some responsibility?  I, for one, am NOT afraid of them and if I had the chance to do it over again working "in-house", I would have the guts to tell them how bad they are right down to their burping, farting, yawning, chewing, mumbling, whispering, non-English speaking two-sided faces.  Let us not forget the 50 second operative note that should have been dictated clearly in 10 minutes.  Yes, they're busy, but the bottom line on this issue is that they don't want to do a surgery and BE TOLD to go and tell someone about it, so they dictate like their hair is on FIRE. IT JUST MAKES ME SICK.  It's part of THEIR JOB.  If they don't do a good job, how the sh _ _ do they expect us to do a good job!

Let us not forget QA.... We are punished and yelled at for leaving blanks........DUH! and I just hate the thought of a QA person (whose quality of work I don't even know), making corrections to my work - and don't tell me that at times, they are guessing... AND WHOSE NAME IS ON THE REPORT - OURS !!!!!!!!!!  In the end, if there was a lawsuit, we'd most likely be the ones to have to answer to THEIR blunders.  I would rather make my own mistakes and have to answer for them than have one or two opinionated QA people telling me what I did wrong... one telling you one way to do something and another telling you a different way.  I say, "leave the stinking blanks and let these JERKS" remember whose country they are living in.

BOTTOM LINE.......... Make the doctors more responsible... after all, they want to get paid and couldn't without us.  I say, "stand up and be heard", tell them to get with the program and most of all, SPEAK DISTINCT AND PROPER ENGLISH!!  THIS IS AMERICA, NOT PAKISTAN.

Please don't get me wrong.  I love what I do and for the right dictating physician (s), it is a gratifying experience.  Something has GOT to change. Thanks for listening... we're all in it together.  We need some mercy...   it's a thankless job  and   !

Someone makes those new now...sm
www.pckeyboard.com its the customizer 101, listed in the unicomp keyboard section. They are $70, but they make them in USB and have a very good warranty.

I don't work there or anything, I just use them.
Wow, really makes you wonder what they - sm
were charging, a dollar a line????
Makes you want to say to such a doc
I don't know. You tell me ... what's a rectalectomy?

I would so love to type the way they actually dictate. I think MTSOs should provide each dictator with a sample of this every few months or so (just as an extra sample) to remind them they really should be more careful.

Even just the way they run sentences together and say is and has exactly the same. Those 2 little things alone are a hassle for us and could be so easily a nonissue.

Okay, I guess I'm going down a different road here. Sorry. :)
Thank you so much. That makes me feel better.
Here's hoping things go smoothly. :)
we are definitely overpopulated and that makes
I live about three blocks from my school, it takes almost a half hour to get there with the traffic, not only in the school to park, but getting backed up and having to sit through two or three traffic lights due to the rush hour.  Rush hour is all day and night here.  Hate it.
what makes you think it will affect us?

What, CEO makes 350,000/year??
that is almost sickening...and what is worse, we are in this mindset that we are supposed to by happy for them, that they should not have to 'share it' with us, and we should be just happy to have jobs and shut up!

I started to hear that in the hospital setting, about 10 years ago, 'just be lucky to have a job.' I used to think back then, well, why, what if I did not have a job...I would be on social services or unemployment, and certainly the government does not want that?! we are supposed to work, right? Life, liberty, pursuit of happiness and all that, or was that all lies?

I know we are competing globally now for work, but does that mean we have to work for nothing now...so even though we have the capability of perhaps fighting for our jobs, we should just let them go, lie down and give up, because that is progress? well, what is the next thing that will be taken? just lie down and let that happen, too? when is it okay to stand up for yourself?

perhaps service jobs (as what will probably be the only jobs left soon) should start paying $15-20/hour.

things are obviously changing, I understand that, but we need to also have a balance. you can not have rich and poor, because that will only lead to some kind of a revolution. people are already getting frustrated, less money in the pocket, etc.

I see these real estate developments go up all over, and more and more, the old for sale signs pop up after about a year as people move in, probably lose their jobs, and then have to move out.
