my WORST docs are AMERICANS who can't be bothered to speak intelligbly enough for me to understand. The ESLs are hard, but after doing them over and over, you can get the hang of it, and it seems that most of them are at least trying to speak clearly, whereas there are plenty of U.S. docs who don't mind pretending they are some country auctioneer and have to speak faster than lightening, or eat their breakfast while they are talking, or chatting up whoever is around them every 5 seconds and expecting you to just sit there and listen, or doing all sorts of other unmentionable things. I think is is frustrating to have an inconsiderate dictator no matter where they come from.
