to send the difficult dictators out (our supervisor did this). That is just part of working for an outside service. We do not get to pick and choose we get what they choose to send.

inhouse work

I am thinking about going inhouse to work for a family practice group.  What is the going rate per hour or per line?  I have 10+ years experience.  It would be working for 2 family practice physicians 25 or so hours per week.  Because I have only done IC the last few years, I don't know what to charge when taxes are taken out. 


Please advise what a reasonable hourly/line rate is .  I am not sure at this point.

Going to work inhouse again
Well, you would have someone to talk to besides your dogs and you might just enjoy the comaradie of other MTs. On the opposite side of the coin, it may just end up being a very competitive, cold and unfriendly place. You could try it and and prove yourself to to be a valuable MT and perhaps they would send you home to work like a lot of hospitals do now if in fact you would rather do that. If you find your home to be a comfort zone and enjoy your dogs, you may just not like working inhouse. It's your call and good luck

I just took a part time inhouse position to supplement my at home income because I am so short... and this is not the first pay period so I am in trouble.

The inhouse position is only part time, but at least I know it is enough to pay my house payment. It is also a lot less per hour, but I want some guarantees so I can stop worrying so much.

those jobs are getting scarce. I would like to know how all us newbies are supposed to start in-house when those jobs have all been outsourced.

I keep hoping some sort of changes in U.S. laws may come to the rescue before it's too late, and the whole industry becomes offshore and automated. Because if that happens, there will be less and less reason to want to entrust one's care to an HMO.
If you cant get a job inhouse, go to a local company and work in their house.
A lot of people who I went to school (college) with ended up working for a local company to Jackson, MS, MidSouth Transcription. They worked with you and got you ....further trained shall we say. We had school, real college courses, so we were very well-trained (had to take an anatomy and physiology class that was the same as the premed students) - we knew a LOT but Ms. Torri got us employable. Try a local transcription company for a while. It will help, I promise.
No (IC)....but I work for a small MTSO and have set deadline each day for my work
I will not be working on Thanksgiving day, though I will have 120 minutes waiting to be done and turned in by 8:00 a.m. on Monday morning.

Hi. I did work for local docs and got ditched by every last one of them for various reasons.....going to voice recognition, going to hospital MT service, etc. Now I work for a MTSO and absolutely love it. The work is there 90% of the time. The holidays are SLOW, but I am learning to try to prepare for that (savings). I don't know if mine needs more ICs (doubt it at the moment), but it is StatIQ Solutions in Albuquerque. They are wonderful to work with. Good luck.

She sounds EXACTLY like the woman I used to work for until I finally had enough of her you-know-what and quit!!You deserve much, much better so keep looking.  No need for that kind of aggravation. 
I work for an MTSO
and I just put it this way last week. I have been working for her for a little over 2 years and have not gotten any kind of a raise. I asked her at what point, if any, does she give raises. Her answer was no one has gotten a raise in quite a while as they had to lower their rates due to overseas outsourcing. I guess the threat of that?

It is the facility's responsibility to figure that out, not ours.
I work as an IC and I let the MTSO know WHEN
I am available to work. If there is work, fine, if not then I don't work. That is a true IC.
The AAMT does not work for MTSO's...sm
The AAMT works for itself, for the few people who make money off of its training material and certifications, etc. and the people who glom on to it that sell MT equipment.  They don't work for the MT's and they SURE DON'T work for the MTSO's!

The MTSO I work for on my own computer
block certain sites when I am connected per their system. The internet is not disabled, but I have to disconnect from them to check my e-mail. However, research sites are still allowed.
I'm an MTSO with MTs who work on a hospital

system just like you do.  The hospital also tried to do that with me - told me they'd email me the production stats at the end of the week for each of my MTs.  I told them no, we wanted to figure our lines independently, and they said okay.  Don't know what I would have done if they didn't.  We work on a Dictaphone system but not doing SR. You say Powerscribe so I assume you are doing SR on a Dictaphone system.  With transcribed reports you can choose the Sessions Statistics tab and get a line count (of course, it is based on what they have calculated as a line but I have checked it independently and it seems pretty accurate).  I have also charged by the minute of dictation in the past.


The hospital we do work for, I don't think they would cheat us; I think they would give us the same production calculations they do for their employees.  But we are NOT employees and we need to be able to determine our lines independently, not have the hospital tell us what our lines were.

I can't understand why people get switched around like they do in other places (I've worked for EDiX and another small MTSO and they switched people like mad).

I used to work for a small MTSO that used gotomypc - sm
I would connect up to her computer and download the voice files to my computer. Then disconnect/log off. Then I would play the voice files on my player (Bytescribe) and go to work. When I was done, then I would just email the work to her. But if email was down for some reason I would just connect back to her computer with the gotomypc and upload the finished work to her that way. Your computer would probably run faster if don't stay connected via the gotomypc. I suppose you can do that of course, just doesn't seem much point to it though to me, unless you only get one voice file every now and then, so you just stay connected to see them come in on the remote computer.
I'm glad I work for a small MTSO....
I hope my boss never sells out like my last one did. My old company went down the tubes after that and I see lots and lots of posts about it here talking about how bad it is. These nationals sound awful.
Before you work yourself into a tizzy of this, contact the MTSO.

For your own protection, contact them before doing any more work and ask for a contract. If they don't have one or want to use one, I would say "no thanks" and move on.


The best answer you will get to this question, however, is from the MTSO. It may just be an oversight as they are apparently busy enough to need your help and perhaps their organization is not what it could be at this point. Your contact will help remind them it needs to be taken care of and would also indicate that you're willing to conduct a professional relationship with them, not one that speculates on possibilities on this board.

If you work for a smaller MTSO, it is rare that
they offshore. I work for a small one FT and another one as an IC, and there has never been talk of them offshoring. A lot of the smaller MTSOs don't.

That's the problem. I work for clinic in day... MTSO at night...
The doctors where I work love my work. They have no complaints and do come in from time to time, so why am I letting this MTSO thing get to me so?  I realize working in the office where I work, the scope of the transcription is not as vast as when I work for the MTSO at night, but it is still pretty intensive. I guess I'm just trying to do too much and I'm totally exhausted. Maybe time for a change, but just trying to make ends meet. Thanks again for your input!

I have my own accounts and I work as an employee, but we are not talking about BEING an MTSO, we are

the MTSO's offer.  If you are a well-trained, knowledgeable medical Transcriptionist with experience and you are looking for an IC job or an employee position with an MTSO, then you are worth more than 8 or 9 cpl.  If all you do is sit and transcribe all day long, you are worth more than 8 or 9 cpl. 


I really don't know what you're getting at.  Not everyone has the capabilities, time, or money to start their own service.  Not everyone is good at pounding the pavement and finding their own accounts.  I'm not a salesperson to say the least.  I lucked into my own personal accounts and if I hadn't, I'd be working for one of the MTSOs, but I'm not.  My full-time employee status job is with a hospital for the benefits only and it pays hourly with incentive.  Right now, that's the only employee status job worth taking is through an actual hospital, NOT an MTSO.  At least at the hospital, you get a decent wage and get great benefits.  Oh, there are a few good MTSOs out there, but they are very few and far between.


I only have dial-up, I work for a small local MTSO -sm
that uses a FTP site to send us our work, so dial-up is just fine for that (work is about a 20 hour TAT, so it does not matter how long it takes for me to download my work, and uploads are quick. So obviously a job that has no set hours, etc. is ideal for someone with dial-up. Also get your own accounts, then it doesn't matter does it as you are the boss. I have 1 account of my own and we transfer the files via the internet through an intermediary website (in place of a FTP). Just keep hunting there are dial-up jobs out there.

I tend to agree.
I am concerned because someone I know transcribes for the doctor I use. But then again, I know the entire office staff, and I have heard the receptionist talking about patients when she was having lunch in a restaurant. Although I hate outsourcing, and I do worry about terrorists sabataging the systems and causing information to be lost, I don't really worry as much about confidentiality as much as I do with local MTs.
DH will be right over. I tend to toss them out, myself.

I agree.  When Angelina Jolie first started bringing to the forefront the terrible African situation, I think that changed the entire focus of this country.  Unfortunately, with that also came complacency.  At least, I give her kudos for bringing up subjects we, as human beings, didn't want to face.  But that was then and this is now.


So...I guess, as just a lowly humble human being, I will throw my hat in the ring and see where it falls.  For me alone, I will feel better just knowing I tried.

Because surgeons tend to be
fast, succinct dictators. Because there are very few drugs to dictate, and the surgeon is familiar with all of them (as opposed to trying to dictate lists of meds as the patient wrote them in a history). Because surgeons tend to do the same sorts of procedures over and over, and the smart, efficient ones learn to dictate the same reports over and over so they won't leave anything out. That means an MT who gets him frequently can make normals to use over and over and save time. If an MT is fortunate and the software is very, very good, she can put jump codes in so she can jump from one particular spot to the next to fill in details that change. An example would be doing cataract surgery, where the main thing that changes is the IOL specific information. Because surgeons usually hate to dictate, so the reports are often as short and efficient as possible. Because it's not psych (or whatever the MT hates most, lol).

Many times, I was called in the wee hours of the morning to type a stat. This was years ago, and it could have been any time. I never questioned my employer's right to ask me to type a stat even when it was not my shift or even when I was not working that day or that weekend. If there was no one else available and the client was calling with a stat pre-op, then somebody had to type it.
Posture. How many of you tend to sit on one leg while working?

I cannot break this habit.  I do not think about it until my knee starts aching midday.  Do you practice good posture?


Unfortunately we as women tend to be raised to think that
our lives aren't complete unless we "have a man". Men, on the other hand, are raised to value their careers, educations and be as financially successful and independent as they can be. Therein lies the problem. We're supposed to dream of a big wedding, they're supposed to dream of being president of a huge corporation.

Moms, raise your daughters to be independent, educated and successful and if someone fits into their lives after they are, that's great; and if not, so what?

No, I tend to hold grudges though I have become - nm
more tolerant in my old age (40, ha, ha). I told my husband only 2 things would ever make me divorce him, (1) he cheated, (2) he ever hit me/abuse. So far so good, though some verbal abuse has happened though much of it is due to circumstances at present but if it continues or grows worse I will seriously consider leaving and I suspect a nasty divorce would ensue (2 kids involved) and I'd be entitled to over $100K....money is what he loves most so it would get dirty I am sure. But hopefully it will never come to that.
My "on" days tend to be when I don't (sm)
have bills waiting to be paid, dishes needing washed, errands to be done or in-laws asking me to do things for them. Not that I stop and interrupt my shift to get up to do those things, but I think the underlying stress distracts me, no matter how hard I try to fight it.

I saw some postings on another board about listening to classical music helping them concentrate. I tried it and it doesn't work for me (I keep stopping to listen!) but that might be worth a shot.

when I read the OP, but everyone seemed to conclude that it was none of her business what the other employee did. However, in my opinion, if the MTSO found out about it from another source, and believe me, she would eventually, it could come back on the other MT as having known about it and said nothing. Everything in this world, these days, is a cover your butt situation. If you knew about it and said nothing, then you are just as much to blame as the one who was doing it....against the contract with the MTSO. Protect your own interests first, and do not worry about the one who is doing wrong. silence is condoning her wrong doing and that makes you (the other MT who knows about it) just as guilty. JMO. So, I guess I am on the side of the MTSO. Sorry.
I tend to proof my reports as I type,
during pauses in dictation I read the paragraphs above and also while spellchecking. Unless it is a difficult report, I do not relisten.
Some who do ops tend to speak faster than lightening sm
I also see it as an excuse for a CYA for them. You have to watch them dictate to appreciate this, there's no need to talk that fast. You almost have to use their boiler plate op as they are unintelligible. They can always say, "That's not what I said." I don't trust those guys. Some ops are great, interesting, some are totally boring and monotonous, all have good and bad points. Psych reports are interesting but if you do them long enough you start to go "batty" - or perhaps it's just me. I like a mix.
