Don't forget you do not have a middle man. Companies get about 15 cents a line US on accounts. I live in the US but am a Newf and have been looking into the possibility of moving and what the rates are for working on my own there.
Anyway, the exchange rate does change between US and Canadian dollars so that is a minor issue but from what I found 10 cents a line is fair and reasonable (Canadian that is) to both parties. The clinic is actually doing better that way as they do appreciate the personal aspect of the person doing the work being the only person they have to deal with. Make that point, they like that. And you will do well as the work will be repetitious and your short cuts will abound.
