I know there are some who are really great and are careful or even just considerate.  But I think for the most part they just think it's a pain to do and want to rush through it and get it done... let the lowly MT deal with whatever garbage comes out the other end.  Maybe I'm a bit bitter, but at this point that's how it feels.

Are hospital dictations difficult to type? I have always typed various specialities for clinics. The company I just started working for lost the account I was hired for. They did offer me a position to type for the hospital dictations. Do you think it will be a rough transition for me? Any advice please! Thanks.
Doctors don't care what's in BOS

I agree, its crap.


And I'm willing to bet the price of the latest version of the Book of Shite that not a single client has even heard of it or gives a fig what's in it.  Most clients have a pretty good idea of how they want their reports to look, and will insist on it no matter what the supposed industry standard is.

I was talking to my mother's surgeon at one of her checkups about outsourcing, and he said their hospital outsources all the x-rays to Australia, because while we sleep, they are transcribing, and the work is back the next day for them. He just smirked and could care less this is taking jobs away from us. As long as it isn't taking his job, it means nothing to him. He's attitude really sickened me.
Maybe doctors don't, but most acute care facilities do.

The changes are in the medical care area. Doctors want to make
They are cutting costs where ever they can. It makes sense to cut costs. But, human error can occur in a place where lawsuits are allowed. I think we will be a safety net for awhile, until a cap is placed on the amount of awards, anyway. I do EMR also. But, they still do dictation a little bit just so they remember who the patient is and can keep the referring physicians well informed.

I just got an account that I under bidded on. My competition one of the nationls (states with an S) but they wanted the work done close to home. Radiology.
Missing dictations
I also get a patient list from the doctors office. I check off which ones I got voice files for, i.e. which ones I typed. I don't keep track of which names or not checked off. How do I know if that patient just didn't show up for the appointment, walked out without being seen, or if the doctor put off dictating. That's administrative work that the doctors' secretaries do at my client's office. If they can't find something they think he dictated and ask me to look for something or check on something I'll do it because it only takes a few minutes but I don't do all the stuff you're doing.

Practice dictations
I am at a hiatus in my transcription career, but I do not want to lose what I know, or lose my speed.  Is there somewhere you can purchase practice dictation tapes, or digital downloads?  I would prefer digital downloads of voice files if there is such a thing.  Just wondering.
missed dictations anyone?
Does anyone have any real experiences to share with the missed dictations? I would like to buy them but I don't want to if my accuracy might decrease with them due to missed dication. although, I think any of my background noise would be louder than any dictator. But I guess on the other end is where the problem lies.
STAT dictations

Re:  Stat dictations.  Does anyone out there charge extra for STAT dictations that come in during the day so that we have to put aside what we are working on and type a STAT letter/document and send in?  This sort of thing can keep one behind in Turn Around Time if it happens on a regular basis.  If anyone charges extra for STAT dictations coming in, what would be a good fee to charge for these besides the line count?

Downloading dictations (sm)

If you key in alt-ctrl-Z while the first one is loading, it will prevent the other two from downloading from the queue.  When you are ready to submit and do another file, you will need to key in that sequence again to allow the next one to come in.

When I got a lot of crappy dictations in my queue, I complained about this and asked WHO assigns the work, who gets the crap and who gets the good ones.
I was told that it is NOT assigned at ALL, the oldest is picked first out of the pool and so on, and that it is all random and nobody is favored.
Now I read in this post that the Leads (team leads ?) distribute the reports. Who are they? The owners, the supervisors or the QAs?
I had once an argument with a QA about grammar and I was right and she was wrong, and from that moment on I got the crappiest crap of the crap into my queue.

Thirty minute dictations -...
that are actually about 5 minutes, once you get past the ummms, and ahhhs, and talking to the nurse behind you, and listening to other people's conversation, and changing your mind 5 times before the completion of a sentence.  Other then that, I can't think of a thing....ahhh, ummm, go back and change....no....under ROS put....
LOL If VR could transcribe his dictations it would be a miracle!

Where to find some Sample Dictations?

Anyone know where I can find a few sample dictations, like WAV files, on the internet, along with the completed typed reports.  Need to test job applicants.  Happy Holidays!

You left out blank dictations
The ones with no dictations - I have a feeling doctors probably pay the MTSO by dictated minutes - but how about MTSO paying the Transcriptionist who has to waste their time listening to them, signing them off and sending them to QA multiple times a day?
Noone can send you sample dictations, definitely

Right on, JMO. Also, when I have the ability to choose dictations SM
from a list (which I don't have at my current work, darn), I like to gather up all the dictations I can find for bad dictators and do them together to learn that dictator right up front, instead of struggling over months. I'll also use them to put together a template, which if dictation's really horrible will include a whole pack of phrases he/she has been known to use, a grab bag of possibilities to scan over. May sound like a bother to some, but it's a real serenity-saver.

I thought after seeing on television so much could get away from this but just had a dictator say, "drowsy and unable to, you know, respond to questions appropriately."

In dictations, I always hear "as well as" when "and" would do. sm
"At this point in time." What does that mean? One says "At this particular point in time." Raises the line count but it is annoying.
Saving dictations on flash drive??
Hi again,  I figured out the settings, and how to recover the files marked done.  Question though on saving the actual dictation.  My files come through email, I was saving in My Wav files and an Archive File; however, the doc tends to send me a lot of duplicates which the Archive obviously does not like. But, also, since I have had a computer crash on me and lost some things, I am thinking of saving my actual dictations on a flash drive and eliminating the Archive file. How long would you suggest I keep them for?  Once typed they go in the Recover folder, how long should I keep those? I found in the help information that we can order foot pedals through Express Scribe and they will be sure that they work with their program, so I ordered one today - figure can't work without a pedal so best have a spare on hand.  Anything I might be missing? Tips or ideas in using the program.

Legal dictations usually are multiple speakers
But, legal dictations usually have a longer TAT.

I learned to put my phones in front of my ears inbetween dictations so I don't get blasted.

Kinship care versus foster care/adoption
Having been placed in a position where I now have custody of my 3 YO granddaughter and going through the legal system, I sought an online network of relative caregivers for children. I would encourage you, especially since you are in Georgia, that if you take any children into foster care with the idea of adopting them, there is federal law that requires the state to take certain actions in a specific time frame. When a child is removed from it's bio parent(s), the state is required to investigate any possible relatives who can take the child before foster care is considered, but even before that, reunification with the parents is the priority. Once a child enters the system and is in the system for 15 out of any 22 months, the state is required to find permanent placement for the child.

The problem with this is that there are case workers who may favor a foster family and do not seek out relative care. I have a good friend in Georgia who had to fight all the way to the state level to get custody of her grandson after the child was placed from the hospital into a foster care home with the promise that the foster parents would be allowed to adopt. She has now adopted her grandson, but it was a long, hard battle to get the state to admit their own interests were placed above those of the child and/or family.

If you get a child placed through the state, please make certain there is not a relative who wants that child before you get your hopes up. The courts are now favoring return of children to relatives even after a child has spent years with a foster family who hoped to adopt them.

States get bonus federal funds by complying with the time lines and being able to close the case, so some states place children in foster care because it is easier than trying to locate relatives.

Didn't mean to go off on a tangent, but I can't imagine my sweet bella going to someone outside her family.
If it was a clinic, it might have been urgent care, but it was NOT acute care. sm
Acute care refers to work in an acute care setting, a hospital, doing at least History and Physicals, Discharge Summaries, Consultations, Surgery notes, Emergency Department notes, and much more, including GI procedures, Cardiology procedures, Neurological procedures, Pulmonary Function Studies.  It goes on and on and it means and acute care hospital setting, not a clinic.

An excerpt fron the site above notes :


"According to the IRS, workers are employees if they must comply with the employer's instructions about their work, receive training or direction from the employer, provide services that integrate into the employer's business, render their services personally (can't subcontract), have a continuing working relationship with the employer, must follow set hours of work, work full-time for the employer..."


 As is noted in the site linked above , the consequences for the employer are quite severe if the IRS determines that the company actually was treating you as an employee rather than an IC.


 


In general,
in a given occupational group the upward pressure that unions place on wages and benefits has the net effect of raising the income and standard of living of poor folk up to the level of middle class folk while the absence of unions tends to drag the income and standard of living of middle class folk down to the level of poor folk. That upward pressure on wages and benefits not only helps union members but everybody else working in that occupational group. Without the organized upward pressure, wages will sink like a rock.

attorney general
good idea.  I have contacted an attorney already (I am a corp by myself so I have access to an attorney) and he states that legally there is nothing right now that I can do. I can possibly sue them later down the road, but they have not done anything illegal. He suggested to try and "sugar" them into paying me.  I will contact the attorney general's office. The company and I are in the same state, so that helps tremendously.

I'm amazed what i can find there for $1. You never know what you're going to find, but cleaning supplies, household stuff, things that are definitely more than $1 even at Wal-Mart...as long as you're willing to not be picky. When the hurricanes were looming, I got mutli pack of name -brand batteries for $1, tons of candles, etc. You just have to search the whole store, but definitely worth it.

It used to be a sign of respect to be a doctor - now they just work for the company store and are running out the door with the nurses at quitting time - throwing any patient they may be seeing to the winds.  One even told me to hurry up and make up my mind because she had to go pick up her kids at daycare!


And so has medical transcription - some guy in a tie said QUANTITY OF LINES COULD = MUCHO PROFIT...


Of course those people don't care if a few people die along the way because of errors. - well not at least until it's their leg that's amputated by mistake.

dogs in general
As a pet owner, I can understand the devestating feelings that you have regarding the horrible death of your dog/child. (I say child because my 2-year-old Weimaraner is like one of my kids) Please remember that any dog can be made to be vicious, there are no vicious dogs by nature. Dogs are a direct result of the environment that they live in and if they are mistreated then they are going to be mean and unsafe. My brother had what is called a Mastif bulldog and it was the biggest baby I have ever see at 100 pounds. Neighbors were okay with this dog in the neighborhood (all their kids loved him) until a picture of his breed was put on the cover of Time magazine. In this picture, the dog was snarling and looked very vicous. My brother ended up having to get rid of the dog because so many people were then afraid, even though before they loved the animal before. There is "prejudice" when it comes to dogs, just as there is "prejudice" when it comes to people.

General Transcriptionist

Im trying to begin my career in MT.  I've ordered a at home study course and im waiting for it to arrive but in the mean time i heard that general transcription is the route to go until you become efficient enough to begin in MT.  Does anyone know of how I need to get started in this and what I need to do to begin?

as a general rule...

I use an "f" after any abbreviation that I want to expand.  I just use the standard abbreviation plus f, so I do not forget what they are.  My ShortHand is full of things like this.


ex: cbcf  tcpf  tshf

General Dissatisfaction
I read all these board and it seems that overall no one is happy where they are.  Is it because this is the most God awful boring profession possibly?  I am so bored that to keep me occupied I find things that could be done better or quicker.  I just switched companies because I couldn't stand same old same old another minute.  Just the new platform and dictators make for a better day.  Am I alone in this thought process?  No, changing jobs is not an option right now kids at home and raising them comes before me..Just a theory

General MT questions
Recently, I was asked the following questions by a friend who is helping to revamp a medical transcription course.  These questions are also asked by individuals who might be interested in medical transcription and are posed at seminars.  
 

I would appreciate your input if you would be so kind.  Also, what is the industry-wide expectation on required number of lines per day and number of minutes per day (8-hour day).

 

1.  On average, how long does it take a medical Transcriptionist to produce 1 hour of dictation?

2.  Besides Word, is it essential for transcriptionists to purchase any of the transcription software that’s available?

3.  Is it common for doctors to transcribe into templates, which eliminates the need for medical transcriptionists?

4.  How many lines an hour should an average MT be typing?

Wages in general
In looking over *** jobs, under the pay/compensation it seems everyone is putting "competive wage/compension".  Can anyone give me some idea of what that means....I am currently getting 8.5-10 cents for IC work.  I have 10+ years and made it at Healthscribe for 5+ years, which was definitely the worst work ever.  Can I do better?  I had my CMT and let it expire years ago.
