the voice file, if possible, and keep my own documentation if it's absolutely a must that you type an incorrect word a doc made up.  Personally, if it were me, I'd type the correct word and not make an issue of it unless it was brought to my attention.  And... if this was brought to my attention, I'd simply tell the person inquiring that there is no such word -- look it up.  One would think that a professional would rather not look STUPID.  It's our job as MTs to be correct and not add made-up words to some make-believe language the docs seem to think they have earned a right to.  It sure would be nice if they would stick to being doctors and let us do our jobs as we are trained.  But it's not a perfect world!  LOL
