When transcribing "command" or "imperative" form, use verb form or "follow up"
For a long time I would become confused by "discharge instructions" type followup/follow up usage ...

Yes, the patient was to follow up with his doctor ... but was also to make a follow up appointment ...

In my experience, certainly your QA may vary and/or be inconsistent all-on-its-own ... but most of these truncated forms, fit into what I was taught was called "imperative case" and follow up is being used as a VERB ...

[[[ even though there is a reasonable argument to be made that "followup with primary care physician in 2 weeks" refers to an "appointment" which is a noun. ]]]

You can figure out the prevailing "rule of thumb" for your QA ...

Not having to think it through --- priceless.

Don't worry abouto nighttime accidents...
A full 50% of 5-year-olds are still not dry at night.  It's perfectly normal.  At 3 I wouldn't even be concerned if he was trained at night.  Nighttime dryness can take quite a while and it's still totally normal.  My DD wasn't dry until she hit puberty, but it DID stop eventually even though it was a long haul.
It is not a plural form. It is a possessive form. SM
The man's name was Metzenbaum and he devised this particular instrument.  The correct spelling would be Metzenbaum's.  WITH THE APOSTROPHE.  AAMT BOS is full of crap.
If the vehicle has been totalled (sm)
by an insurance company it will have a salvage title, so those will have evidence that the vehicle was totalled. I have heard of auto salvage companies using creative measures to disguise the real damage to the vehicle (like pounding the body with a hammer to simulate hail) leading the buyer to believe there is only body damage. We bought a submarine once and it was the best deal we ever made, a year old full sized fully loaded truck for less than half price. It was river water though and only 3 weeks after the flood. The only problem with it was the cruise control didn't work! Anything out of LA or MS at this stage probably already has rust.
I'm wondering now if you use a vehicle...
for easier application or do you just use straight water? I got quite a bit of the powder in my eyes, etc. I'm sure I could get used to this and even improve my technique over time, but I just wondered. I've read about using just about anything from honey to soap to apply it. BTW, my pores look better after 1 use. Thanks!
Hybrid Vehicle

Yes, I love my Insight.  It is 2001, never had any problems and has 50 to 60 miles per gallon of gas usage!  When I purchased this one, I received a 15% off the top for the 21K cost of the car on federal taxes.  I could never go to a regular car again.

The best part is:  the mileage is even more because the engine shuts off at a stop sign or when in road rage traffic.  This would prove to be even better mileage since all other automobiles are steadily burning while this Insight is completely off.  Of course you can adjust if you really need air conditioning.  It came with auto everything:  windows, transmission and power brakes and steering.

leasing vehicle - sm
Thanks for all the reponses, honest and otherwise. You all basically told me what I felt inside - the only one who makes out on a lease is the dealer. Thanks again.
leasing vehicle
If you travel 12-15,000 miles or less and do not want to worry about repairs, etc, and know you want to drive a newer vehicle always - having a payment always, leasing is good. I leased for a while and it was fine, but then started putting on a lot more miles. So now I buy my vehicle and drive until it dies. Matter of opinion.
I would think that owning a vehicle with low miles is a plus - sm
at the end of that 5 years you own the vehicle, versus leasing you don't; and you would owe a huge chunk if you choose to buy it. After 5 years of paying a car off with low miles, you can choose to re-sell and get a better price because of the low miles (big selling point). So whoever fed you the line above is, I think, totally off the mark (or trying to lease you a car).
Husband bought a vehicle, in his name,
husband died. Money still owed on the car. I did not want, was not responsible for HIS bill and nothing they could do. Told them sitting in my backyard, come and get it. I did not own, had not signed my John Henry on it, so glad. Could be similar if you are not signed on the house.
anyone have personal experience with leasing a vehicle
I've never owned a car, end up upside down on the loan, owe old loan on top of new loan, and own it for so long it starts to tinker and I pay for repair on top of the loan.  What is the downside of leasing?  I heard you sometimes have to pay, but only if you drive over the limited mileage right?  Working at home, that saves mileage on commute which makes up 70% of it I would suppose.  Plus being new it won't nickle and dime me to death with repairs, low payment because you only pay for the amount of money it would depreciate for the time you own it.  Any personal experience with this, and is there red tape or hidden small print?  Going to start looking really soon, I only hear bad things but its from people who drive a lot.  I think a new car is a necessity in this day and age, with all the swindling mechanics and the dependence to get from here to there.  A new different car every three years doesnt sound so bad either.
Anyone lease their vehicle? Need pros and cons please - sm
Am considering trading vehicle, and was advised because of the low miles I put on vehicles (under 25,000 miles in 5 years), that I would be better off leasing than buying.  Would appreciate any and all thoughts and things to watch out for when leasing from those of you that have gone this route.  TIA.
leasing in like renting - paying someone else for the privilige of driving their vehicle..sm
with the low mileage you put on vehicles, you would have an easy time selling outright to a third party. Crunch the numbers.  figure out what you would pay for leasing, including deposit. Figure out what you could buy that same car for. If you hold it for 5 years, you can get an idea of how much cars depreciate by checking blue book prices on the current model and a 5 year model. Use that to figure what percent it would depreciate. Then add your leasing costs all together. Add your car payments all together.  Then subtract from the car payment what your car would be worth in 5 years, because you'll get that back, ya know. IMHO leasing is done by people too lazy to figure out the numbers and by people that it's important to "look good" to the neighbors, regardless of what your bank account actually holds. Hope you don't mind my honesty.
Here in North Carolina they get to operate a motor vehicle at age 15 when accompanied by
a licensed driver.  No, they do not state WHO has to be the licensed driver (i.e. friend, neighbor, etc) or how long the licensed driver needs to be licensed, but at 14.5 years they start driver's education in the schools!  It is ridiculous to me, as they are children operating a very dangerous toy and I pray they up the age before my children reach it, as I do not feel children possess the maturity and forethought to drive at such a young age.
It's when a vehicle is reconditioned, for example, new paint, fancy wheels, stereo, upholstery, D

Form

I got the form and started to fill it out. Where do I get another one? There was a phone number and web address on the information but I'm trying to search and can't find anything. Help hugs

The new tax form

so as IC/SE anD EMP..which form did you use..
I was SE for most of the year and employee for 2 different companies last 2 months of year..I also am now part-time emp. with a co. and trying to start up my own business which I started up in Dec. and want to take deductions for equipment purchased for that business. Oh my head is hurting.
Is this form something I can...
request to fill out on my own, or is it usually requested by the IRS?  Will the company I contract through know that I have requested this form?  I, too, am an IC who is treated like an employee.  I have worked a set schedule (days and hours), including nights and both weekend days, for a long time with little or no flexibility.  The pay is good, though.  TIA
Form your own.
I love escription...won't work on anything else.
There has always been some form of
QA, don't know that there was ever an official start. Most employees in any business are evaluated on the quality of their work.
You don't need the form.
You don't really need the form.
You're funny. Ignorant, but funny.
I don't live in NYC and I don't work out of NYC and I make wayyyyy more than $23 an hour, SandyGIRL.

I'm an employee at the largest national and I make anywhere from $58K to $64K a year.

You don't have to believe anyone but you show your ignorance by telling those of us who make that money that we're full of it.

Hahaha
Sorry, that was not an ASR funny, just a regular ole dictation funny. LOL! nm
.
continuous form
It probably means to just type the dictation and make new paragraphs as needed, but to just keep going down and make no headings unless specified.
I think that any form of indiscretion with someone
else would be considered being unfaithful to your spouse. That includes talking on the phone or in person when your spouse is not aware of it, holding hands, kissing, or "going all the way." Anytime something is done with another person and we feel it necessary to keep it from our spouse, there's something going on that doesn't need to be. This, of course, is just my opinion, but I hope it helps.
Just tell yourself all of that is just food in a different form. lol sm
Seriously, though, just put in your contract you don't do toilets, the same as some don't do windows.  I bet there are people who would rather get other things done, and would be willing to do that themselves, especially if the price was just a tad bit lower.
form 8829
You said you use your den "almost" exclusively for business.  Almost doesn't count.  The room has to be used EXCLUSIVELY for business in order to be able to take business use of home.
would you say its the same as "paragraph form"?
thanks
why form "friendships" - probably so that the gov...
can dump more money into the pockets of the Indians! Did you know that the government will give out $60,000 loans or aid to help Indians/Pakistanians help open up a business in this country!! I couldn't get a business loan for $25,000 and I'm from this country!! Born and raised!! There was a grocery store in our town that was for sale and no one in town could afford to buy or get a loan for it, then out of nowhere, this Indian family moves to town (lily white community, by the way) and bought the store. They told the store patrons that the government gave them the money to open up a business. Can you believe that???!!!

No form needed.....
Just put your complaint in writing and send by fax, mail or hand deliver to the Patient Care Representative at the hospital.

The PC rep at the hospital I work for investigates all complaints from beginning to end and will make a follow-up call back to the patient or patient family regarding the issue if further comment is wanted.

I've typed these reports for admin and can be very interesting to say the least.

it is a form of media ...
for healthcare professionals.  It has a circulation.  It is readily available in schools.  All media matters because what is in the media influences the way people think.  I think it gives a negative view of us to those who call the shots over us who read it.  That's why I think it matters.
Would you capitalize the name of a form the doc's use?

20s and 30s is the correct form. SM

BOS says:

"Add s without an apostrophe to form the plural of multiple digit numbers, including years."

However, the BOS also says to spell out nonspecific numeric expressions, i.e.

"She described hundreds of symptoms. (Not 100s)"

So I don't know what to tell you to do.  You decide and go with God.

Well on the form in the book from DMV
Well on the form in the book from the DMV it is listed and the doctor is required to check. By the way my daugther has a lady doctor, not a man.
We all need to JUST SAY NO to VR in any way, shape, or form
Having to edit some of this garbage is more time consuming than typing the whole report not to mention the looking stuff up and verification of names, etc.  It's all a big RIP-OFF.
They won't take out taxes. Did you submit a tax form? nt

I have a test for a position and it says to do it in continuous note form, does that mean that they should be all in one document in Word or am I completely wrong on that?  Someone please help soon!  Thanks!
continuous note form
Are you testing for MT group? I took the test and just typed the patients one after another. I kept spaces between the patients with the patient's name.

An account has asked me to prepare and sign a HIPPA compliance form. Please refer me to such a form.

Maybe we should form a support group

"Rad MTs Anonymous" - LOL

But hey, anyone who has participated in this thread who had a similar experience to mine is welcome to email me. I would be happy to swap stories, offer support, and just generally help you (and me) feel that we are not alone in this.

Thanks to everyone who responded positively. Your support is greatly appreciated.

In letter form for clinic?

If the clinic is consults in letter form, that slows me down, looking up all the docs and addresses.  Not all clinic is like that.  But that part may make a difference.  I just quit a company cause of that and went back to acute care.  I have also worked clinic that we didn't have it in letter form, and it was fine.  Just depends on the account.

I would think it be depend on the type of form.

ZyloMed 1099 form

Ex-ZyloMed IC wondering if there are others out there that still have not received their 1099 form in the mail.  Wondering if mine got lost in the shuffle somewhere.

Homeland Security has an I9 form for all
There are several documents accepted but if the employer wants the SS card and/or DL as those documents, they can ask specifically for those.

what gives on 1099-MISC form?
The MTSO I worked with last year as an IC says they just got their 1099-MISC forms and are in the "process" of mailing them out.  I checked on the web and it "seems" that the government has extended the time to mail them out to 02/15...anyone else know of this?  I can't do my taxes until I receive that blasted form.  GRRRRRR!!!!!
yes you can....dont' really need that form to do your taxes. nm

"Thecal" is the adjectival form of "theca"
Maybe you can figure it out from there. Or, you could just look it up in your medical dictionary. I did, and it's all right there.
