It may be that...

the MT is actually using VR but is not telling the MTSO about it.  That could explain doing 500+ lines per hour consistently.  I generally transcribe in the 350-400 lph range doing straight transcription, but I also work on an account with a lot of templates and also for an MTSO who pays honestly, which means we get paid for every single character we type, headers and footers, etc.  That makes a big difference.
