 that you would use over and over again -yes they go bad.  You will know it when the voice on it sounds distorted and is intelligible.  This is a nightmare when it happens, especially if the doc doesn't use a numbering system to know what pts were dictated on that particular tape.  Most docs will catch this because as they are dictating -stopping and starting- they will rewind the tape a little to hear where they left off and will hear for themselves the distortion.  It's a good idea to refresh your batch of tapes with new ones peridically - when you feel you have used them for an extensive period of time.  How often? You'll have to be the judge of that.
