follow-up (adjective)= follow-up examination

followup (noun) = return for followup

follow up (verb) = the patient will follow up

(in regards to not hyphenating and doing just 1 word, I would think that would apply only to those people who follow the rules in BOS...and I think most of us do what the account wants, not BOS)
