Depends on the company. Some companies require it; others do not. Some don't require it
if you promise that you will seek your certification at some point in the future. Other companies could care less if you are certified at all at any time.
Depends on what kind of hospital? Large urban hospital or small community hospital? SM

Also, is it a large teaching hospital? If so you have to consider there will be A LOT of different residents dictating, usually a lot of ESLs at teaching hospitals, and the residents rotate out and new ones rotate in every summer. So you can't expect to get the same dictators and build up your macros because the dictators change all the time.


I would say 9 cpl would be a pretty good offer for a small to medium community hospital where you will be doing the same dictators on a daily basis.  But for bigger, urban or teaching hospitals I would want at least 12 to 15 cpl.

What you describe is not unique to MT

I've worked in manufacturing (production control & customer service) for many years and realized that EVERY company, EVERY manager can do a better job at motivating and communicating with their employees. However, many managers do not know what to do or how to. They have their own managerial style and they “apply” that to everyone they supervise. A good manager will figure out each person working for them and determine what novitiates that person. Some like to be praised in public. Others want a private encouraging word. Each person is different and has different expectations. The grass is not always greener on the other side. I’ve learned that too many times in my career! BTW, I am considering enrolling in an MT course and read this board and others for information to help make my decision. This was a topic I just HAD to chime in on because this is true of ALL companies… Wal-Mart, GE, Walgreen’s, Ford, Kroger etc. Looking at the big picture, doesn’t it all boil down to how good are you at relationships and making them work?????


We are one of a kind, very unique, not just
anyone can do our jobs.  No matter what your situation, children, caring for aging parents, raising animals, raising crops, tending to another at-home business at the samee time, we are definitely jugglers, no matter how you slice it.  Sometimes I get on this board and think, ahhh, I know I have been there, done that, and seen it all, but I really haven't.  Hayseed, I think you are a riot!  There are so many others as well with just great informative posts that I'm glad they took the time out of their day to share, whether it be a funny or just to vent because it gives you a place to call "the water cooler" when no one else can really truly relate!  Thanks coerts, friends, and fellow MTs for all of your input.  I always new good things happen in numbers!  Here's to ya'...    Back to the  now!
Unique situation

I have two part-time jobs, one for doctors I worked for before in another setting - now I am doing their overflow work. The second part-time job is with a local MTSO. It is the only account they have with my specialty.


No, I don't think I could do autopsies, although I love watching "Dr. G., Medical Examiner" on Discovery Health. I close my eyes during the gross parts, though.

I am not sure, I have a unique situation, but sm
My problem was no mortgage and being an IC. In October of 2007 I filed my taxes for 2006 (had an extension) and I got sucked for $7500 and then another $400 for penalties. In April of 2008, I got sucked for another $7600 and another $350 in penalties. There went my new car. There went paying off a debt I have been years working on. So, no I don't know how you do it and pay your taxes!

In 2008, I went to work as an employee. They will pay part of my taxes, they pay a fair portion of my health insurance premium, I have PTO and everything else. I was thrilled to work for only one place, till my tax accountant told me that in my circumstance, I would not be able to deduct any of my expenses because I can't itemize my taxes! He told me to work at least PT as an IC. I did for a time Jan-March and then quit. By May 1,2008 I knew I had to look for something. I ended up going back to work for the place I had quit in December 2007 to work for my employer. It turns out it is a 6-hour day on top of my 8-hour day. That was not the agreement, but it is the reality. I will be able to file as an employee and a Schedule C as an IC. I'll have my cake and eat it too.

I am saving 1/3 of after tax income, but I have very little free time and only one day off a week, if I have that much. I will meet my goals for a newer vehicle and paying off my debt next year, and that is great, but I will have had very little fun otherwise. I don't even have time to grocery shop most months, but I'd have money if I could get there. I am known for being a total workaholic and it is true.

The only thing I can tell you about making it all work is to scale down, down, DOWN! I went from a 2400 sq ft house in a large city, to an 1100 sq ft mobile home in a small town that has DSL. I bought the mobile home outright for a paltry $3K and while I don't own the land, my rent for that is only $75 (hence my tax problems). Where I live, it is so rural that the nearest Walmart is 60 miles each way, but it is best that Walmart and I should be parted that way. I get there perhaps 3 times a year, which is plenty. I eat at home. I make my clothing. I clean my own home. I do my own laundry. I pinch every penny I can figure out how to pinch, and then I pinch it harder. I gave away much of my furniture in this move and replaced it with second hand furniture that fits my new smaller home. The house was a real fixer upper and I have almost completed it...on a shoestring doing most of the work myself (but not this year). I grew up poor, I was poor when I was married and raised my kids poor. I am no longer poor, by the standards of where I live I am quite well off, but you would never know this by how I live. I run around in comfy sweats that are old, that is why they are so comfy! I live in a tin can and in a dirty little town full of wonderful people. I happen to be in love with my tin can which I have transformed into a very cozy, inviting home where guests love to visit...for tea in cups and saucers from the thrift store.

It is hard to make more money. I knew my energy limits and what I could and could not do. I made a conscious decision to work the money equation from BOTH sides...spend far less and make as much more as I felt I could. The end result has been better than I had anticipated.

As for getting out of MT, we are all pretty well in the same boat if we have been doing this for a long time. You might try a job center through where you live. Have them help you brush up your face to face interview skills, and see about how to present yourself as professional and capable. We have all been out of the fix your hair and makeup, actually get dressed and leave the house mode for a long time. I know if I were going out the door to work, I would need to follow this advice myself. I need some fine tuning, possibly so do you.

So stay in or get out, that is up to you. I thought I would give you why I have stayed in and how I did it.
Since that would be unique to each company, I don't know how anybody here would even have a clue.

We went for counseling and boy did things change!!!! We now listen to each other. We talk more than we ever have. We do things as a couple, for each other. We learned to openly appreciate each other for little things. We learned to love each other in ways that we never did before.

Believe me, my DH came from living at home with momma into our marriage and he expected me to take the place of his mom. That worked for a little while until I grew tired of being his maid and his bedroom companion.

I read that this story is so unique which is why it is getting so much coverage, and her parentes are talking to the media which a lot of grieving familes won't/don't do. Think about Jessica Lunsford's dad and how much you saw him on the news talking all the time to try to help his case. Which is what Beth Holloway Twitty is doing.
I truly think that if this Holloway gal was black and her parents were willing to talk to the media and give interviews all the time, I'm sure there would be coverage.

It's not about a rich white thing. Get over that. I am Hispanic and I don't even believe that. If you had a family member disappear and you were willing to talk to the media and had a good "story" you would get coverage. Trust me.

broken out in red blotches that are very itchy.  He took some Benadryl to relieve the itching, but what could this be?  Is it something he needs to see a dr for urgently?  TIA

There's a rash on my hiney (referred to hereafter as DH -- dear hiney). It's getting splotchy. Can you tell me what it is? Also, do you think it was something I caught or do you think it was something I ate?

Should I call somebody? Do you think I need to take DH to see a doctor?

Fairly new mom

Things not to buy -  When my son was born, we got 40-something blankets, 30-something burp cloths, and way too many 0-3 month outfits.  While I was extremely grateful for all the gifts, I had to spend a lot of time exchanging the multiples.  Amazingly, we had to exchange a lot of diapers, too, because the cheaper ones don't always work as well on little boys. 


Things I loved were anything homemade, personalized, etc.  Also, outfits, socks, and onesies in bigger sizes, 3-6 months and 6-9 months, were a lifesaver and money saver as he got bigger, which seemed to happen overnight.  You can never go wrong with wipes, shampoo, and other essentials.  My son is 1-1/2, and I have yet to buy any shampoo or lotion for him.


I am fairly new at this,
and I have made the mistake of using palpations before. I know the correct term, but I just made a mistake. As soon as I see that mistake corrected, I realize I messed up.

I do not see how someone with 20 years of experience could be making those mistakes and think they are correct. I have known those proper terms for a long time now. I just cannot see how they would make it this long and never once be corrected on them.

Here's one fairly easy
I was once a "head on a silver platter" - Got a big square box top, bought some cheap fabric enough to drape over the box, cut a slit and a hole in the box top enough so it would fit over my head. Then I also cut the fabric to drape around the box and decorated my face up  in white face paint with red lips to look like the blood had all drained out of my face. You can embellish it anyway you like. I made the platter out of a bid cheap silver tray like you get at Thanksgiving to put the turkey on and a I pasted plastic fruit around on the platter. The point is just to make your head from the shoulders up look like it has been cut off and put on a platter. Everyone liked it. I could email you a pic later if you would like. It was really fun.

I thought I was fairly good at them
got several that I absolutely could not understand 1/2 of what was being said. Made 100 lines in 3 hours!!!!
It's easy if you are already an MT, but you really need a fairly extensive...
knowledge of anatomy.  CT scans, PET scans, MRIs... Knowing your anatomy really helps you catch mistakes or know that you're not making mistakes by typing the name of a body structure that isn't even near the area of that particular scan.  At least where I work, if you make mistakes on x-rays the radiologists get REALLY TICKED!  They read them all and when they find the smallest error they're on the phone to the transcription manager, and we hear about it.  Doctors are making medical decisions based on the findings of these x-rays and scans, so you can't exactly be making errors.  But, if you have a good medical transcription background, you can be trained pretty fast.  I had never done them until recently but learned fast.

I don't think you could fairly penalize an MT for not understanding a word without (sm)
without also penalizing the dictator for not being able to speak the word. 
Had a fairly difficult time adjusting but,
I believe it is doing my hands and wrists some good. The keys seem more difficult to depress than previous keyboard, and it sounds like a rattle trap (plus the p sticks), but other than ALL THAT, I think I like it.
I ask for a raise fairly often. The worse I can be told is no and sometime I have gotten a yes.

I've been using Dragon fairly steadily

I have excellent results. Maybe one or two mistakes per page, if that much.

Any number of factors could be the reason for the problems you're having, starting with your soundcard, improper training, not dictating correctly, etc.


I would be proud if my records, with my name erased, could be used to help an MT be treated fairly.

Starting pay is 6 cents a line, fairly good account stability.
I had lots of ESLs. Benefits are okay. They have a new, stricter QA policy in place with new compensation, hard to get a 98 or above as it is by the BOS. It was okay, but couldn't make any decent money. Need to be a super high producer.

into the "global" normals, I will play with it again and give it a try. How long have you been doing transcription? Have you stayed with the same company for a long time? I worked at a clinic for 6 years, hospital for 8, and now this new national for 3 months.....I hate feeling new!

Summary of requirements: Work environment must include separate room in the house with a door that closes. Workstation must be free of any noise/music/TV or other distractions while working. PC password protected including locking screensaver. No one else in the room while working unless they also work for this company, etc. Dunno, maybe working for a hosp at home has more strict requirements than some of you.

even though DOL states you must be paid for all mandatory work performed.  If you were to volunteer to do it, that's a different story, but we are REQUIRED to do all this BS clerica/bookkeeping for the hospitals, demographics, notes on discrepancies, CCs, look up names, bombarded with e-mails, phone calls, reviewing account specs every time an account is changed, told to do "exhaustive search" if name cannot be found (easy for THEM to say, isn't it?).


I could go on and on and on.  VERY PROFITABLE for the MTSOs, who use it either as a selling point to keep the account or are actually being paid for it. 


Which companies require ULD???

I'm looking for a job right now and obviously can't afford this headache (or added cost!)  Can you give us names of MTSOs who you know "require" LD???

then they should work. If they are happy doing without many things, then no. It sounds like that friend of your daughter has FOUR very irresponsible parents. Not her fault. Another product of divorce it sounds like. I have read that many parents of divorced kids spoil them rotten out of guilt because they broke up the family. Sounds like you got it together, though! ;)

one way or another. Some will hire w/o a test based on your resume and experience, and will know rather quickly if can do the job or not. I have gotten 2 jobs that way, one as an employee and another as an IC, however this was after I had a minimum of 3 years experience for this ever happened to me--at 6 years now. I am still with the IC one (since 06/2007), had a falling out with the other one but did work for them for about 9 months and they were happy with me; left as I was unable to continue working a firm schedule due to change in circumstances and they needed someone to work a set schedule, so we parted ways. But as a newbie no one will hire you w/o a test. So just keep plugging away and test a lot, they are a very good way to learn what to expect.
Most state they 'PREFER' a CMT, not REQUIRE.
I think the reason that's put on most job ads is that considering what they're paying, they're going to get a lot of applications from newbies. The companies THINK the CMT guarantees a good employee, though this isn't true, either. If you're an MT professional that's been around a long time and can do the work, you are still, without those three meaningless letters, more of an asset to any MT dept. or company than at MT of lesser experience WITH a CMT.

Another take on that: Often those of us without CMTs have been around longer than that dubious organization has, and we know what MT is REALLY worth, because we used to be around back when it was a respected position. Lots (though not all, I realize) CMTs are younger, have been in the biz a shorter time, and have been told while they were in school (and possibly a school associated with AAMT/ADHI) that a CMT was 'necessary'.

Nowadays even the best national companies out there pay 'rinky-dink' wages, which has nothing to do with whether or not an MT has one of those useless 'credentials' after their name, and everything to do with greed, and competition from cut-rate transcription companies in other countries.
In future, require a deposit sm
equal to the cost to replace the equipment and put in a contract that the deposit will be returned within 2 weeks of receiving equipment in good condition.  Anyone who truly intends on returning the equipment will have no problem paying a deposit, especially when the terms are in writing.
Most companies these days require even
FT employees to work at least part of the weekend. I tend to work almost every Saturday, 3 to 4 hours. I am a FT employee working a 40-hour week and just take off early a couple of days during the week.
do the Bose QC2 require batteries?

You know it doesnt and neither does any other state for driving nor for working. This person sounds like not only under stress but more going on and perhaps she should see a doctor.
The industry does not require QUALITY because
there is no reimbursement to the hospital or the physician for the actual transcribed report. The insurance companies are requesting written documentation but do not pay anything for it. This is why the hospitals are willing to settle for the cheapest rates.
