It is like any other set of dictating physicians, some are good, some are wonderful and some totally suck.

I made the big transition from clinic for 8 yrs to acute care 4 or so years ago. I went right from clinic to typing only OP notes. I won't lie to you, it was a very rough 3 months for me in that transition. I worked like a dog and I put in many many MANY hours at it to get any kind of lines. At 4 months, it suddenly got a little easier. After 5 months, I was transcribing 1700 and 1800 lines a day in less time than I took to do half that in the beginning.

Don't let this scare you off. Acute care is where the money is right now and the type of work that will last longest because I think clinic is going to dwindle and be very slim (personal opinion). I literally went from doing family practice for 8 years to typing OP notes and I did it quite literally overnight. I didn't have DS, ER, H&P, or consults under my belt. I went from some of the simplest to some of the hardest. It worked out in the end and I have almost not typed anything but an OP note in the past 4 yrs, and I have respect for clinic MTs because I can't do that anymore, not that I wouldn't, I CAN'T.

If you have done a lot of clinic, you really can do H&Ps, ERs, DSs and consults. You already have the ability to do these and I think you would do well with them, and enjoy them even. With some determination on your part, you can get up to speed much more quickly than I did because I skipped this step. If you are looking to do OPs your difficulty is going to be finding a place that will allow you to learn a few at a time until you get it. These days, from what I have seen (not everything for sure) many companies are going to hiring OP note MTs to do OPs and hiring others to do the rest of acute care. It makes it grand for those of us how love OPs and it means that those who don't like them or have not learned them, can still be in acute care and make money. Win win.
