OMG! And here I thought doctors couldn't surprise me anymore with the dumb stuff they say but good God! Yeah I would leave that blank. Hey we don't get in trouble if we have to blank something but personally I would not be a party to that myself. If he wants to be an arrogant jerk then he can bloody well type it himself! Good call.
