It sounds to me like you would not be losing much if you lost this account. I would go in there and lay down the law with him.

* No more waiting until HE has a minute to go over the reports. Agree on a set time of the day to meet with him and make any corrections. DO NOT wait in the office past the time it takes you to put in their notes.

*Put doctors names in your Expander as you verify their correct spelling. Make a code for them so you don't get them mixed up with something else -- start all names with a number or something like that. If it is a new doctor and he doesn't take the time to spell it for you -- his loss. He can wait til the next day to get that corrected.

*complaining about the ink?? HOLY COW. I just don't even know what to say about this one.

*change your pay scale to a 65-character line so he is ONLY paying for exactly what he can see on the page. This nickel and diming about 1/3 or 1/2 page is just BS.

*you need to either raise your rates or charge him a trip fee. You should probable be charging him an aggravation fee -- but that can be disguised as the "trip fee!"

If he is going to be a PITA, then raise your standards. He will either buck up and pay it or fire you. If he keeps you, he may even gain some respect for you. But if he fires you, will you really have lost much??
