My experience has only been with smaller MTSO's, and in some cases it had to be done or lose the client.  I have known of clients requesting that a certain Transcriptionist never do their work, even though that transcriptionist was a good transcriptionist.  The answer would not be to fire an otherwise good transcriptionist, but simply to assign another transcriptionist to this account.  I know of one instance when a physician asked that a certain transcriptionist be taken off his account, when another physician asked that when possible only this transcriptionist do this work.  Small MTSO's also assign accounts based on the MT's experience with a certain specialty.  Many MTSO's assign work preferentially to certain MT's they know they can count on to get the work done, rather than send work to an MT who tends to not meet turnaround time, or refuses to work certain hours.  Although it is not always done unfairly, it is definitely done.
