Just typed my 1st facelift report and I must say
I'll never have one done. It has to be the grossest report I have ever typed.  When I do op notes I find it very helpful to visualize in my mind what the surgeon is doing. Well, with this report, I was ready to heave all the way through it. My face can wrinkle and sag til the cows come home cause I will never have facelift done. YUK! 
operative report vs procedure
I used to think I understood the difference between these two types of reports (operative versus procedure). Somehwere along that way, amidst dozens and dozens of account profile specifications, technology advances and MDs who always key in the OP code regardless of report type, I am not so sure anymore. I would like to try to elicit a consensus on how transcriptionists distinguish between these two report types and what criteria they are using to do this?
Operative report question...curious...

What is the most popular or should I say proper way of setting up a heading for an operative report, order, spaces, tabs etc?  I know the doctor's way is always right but some doctors do not give enough information and some give too much.  Just wondering if you tab over diagnosis and align or place list under headings.  And spaces after each heading.

Softext MT CD of Operative Report Samples

Has anyone heard anything about this CD?  I found it on ebay.  It has over 1000 multispecialty operative sample reports.  It's called: Softext Medical Transcription CD of Report Samples.   eBay Listing: 180302333955. 


Sounds like it could be a really good reference.  Just wondering.

I know for a fact in order for insurance to pay there has to be a dictated report.
No insurance payment without copy of report in office where I did billing.
How long should it take to type a 36 minute dictated report?
I was just wondering as I talked to a woman that said it took her a really long time to type this report the other day.. I have never actually timed myself, but thought I'd see what you all thought or what it really should take to do a report that length?

Pretty interesting how they get that skin pulled up underneath the scalp.
In my first post stated I had facelift years
before this one, they only last for about 10 years or so and so this was supposed to be a repeat. I had scars, just not complete from this person. He made the incisions around the ears and told me forgot about total lift because took out Gore-Tex around nasolabial folds I had inserted prior to get those darn lines out. This was not him that put those in, by the way. I did have previous scars though.
Operative Notes
Does anyone receive higher pay for doing only operative notes? I am just wondering because my account paid last because it was ER reports but now I am doing almost all operative reports. Thanks for the info!
Can any of you MTs who type operative reports tell me
what are some good websites to look for spellings of surgical terminology, instruments, equipment, etc.?  Much appreciated!!!

Hi everyone, I was wondering if anyone knew of any good websites for medical equipment and/or websites to help with Op notes.  Much appreciated!! 


Operative word here is "should"

We all know they should, but my point was most won't. But then again, I do remember instances where it was addressed with some of the worst ones, and they improved for a while and back to the same thing.  One radiologist everybody hated typing had a very thick Latin accent, and I ran into him on the elevator and he asked me if I type his dictation, to which I replied in the affirmative, and he said, "Good luck to you, brave lady"!!!!!  Then he would have the audacity to complain to our supervisor that we left too many blanks, that he says the same thing every time, why do we leave blanks!  The nerve!  She did tell him that he was difficult to understand, but do you think he changed?  No.  One of the reasons we could not understand a lot is because he would turn the x-ray films as he was dictating, which would be very noisy, and then he would have the machine on speaker and turn his head from side to side as he'd dictate.  Nice!  But did he stop after being told this?  No.  Nuf said, really.  Just one tiny example.

They have no clue what we go through and they are cheaper than their husbands. IMHO they should stay home!
Mastectomy operative reports
Does anybody out there have mastectomy operative reports that I can use as samples? Or can anyone direct me to a surgery site that would have this type of report?

This is a good one. MT Daily has sample reports. Another one is mt-stuff.com. These are just two good ones. I have done Ops for 30 years and if you need help, let me know. Hope this helps.
Someone left out the operative reports
those are certainly considered in the 4, not ERs.
Same here, I like operative notes, but rarely get them, by the end of the day (see inside)
I am fuming. I make more money on operative notes and am motivated. Discharge summaries are hard to make money on for me, as they are all different.

I totally agree. Operative notes are more complicated and should be
paid at a higher rate. I know it takes me longer to do them because so many different surgical pieces of equipment can be used etc.  Anatomy has never been a problem but mainly the different surgical equipment items. I would love to find a job that offered me more. I just dont know where they are.  
You do not need the 1099. You just need to report the income. Report the company/person ...sm
to the IRS for not sending out the 1099. 
Why do so many people LOVE operative notes? I see posts about it all the time...
how they make good money on them, they could do them all day, feel cheated when they don't get them? I have been doing this for years and years and still kind of groan when I see them come up on my screen. Fill me on the secrets and maybe I won't whine so much when I do them. Thanks gal!
I charge the same amount for a "normal" report as for any other report.
You still have to listen to the dictation and change anything that's different.  I had one woman try to pull this on me.  She'd dictate, "Just pull up my normal, but change this, change that, switch that around, move that, add this, delete that, and change the other."  Then she'd only want to pay me what equated to $.03 per line.  She wanted 1:1 on her dictation to transcription ratios.  I told her to take her cheap account down the road because I'm worth more than that.
Acute care work is operative reports, consultations, H&Ps, emergency room, DS basically the type of
dictation found in a hospital setting as opposed to a clinic setting in which you just type office notes and minor procedures.
Oh, a report just came in. A report actually just slid in, can you believe it. Hip Hip Hooray. I

had better get that sucker typed before it gets out of ONE MINUTE TAT.

If you are careful with putting the correct report in the correct report shell and patient, you will
not have any problems. I only take away this option when someone is careless. There can be NO room for error on this. One mistake can be very serious. Many do it well though, so just double check and you will be fine.

I got approached about possibly doing the transcription for a local doctor office - 4 doctors.  I have DSS player and they have used DSS system before, but how would I get the files from their computer to my computer.  Could they load them into a folder and then winzip them and sent to me? Is there an easier way?


I know this was discussed before. I had no idea then I might get this opportunity, so I didnt pay much attention to the post. 


ATTENTION TYPISTS!!!!
Back in the 1980s there was a doctor in California who used to start each dictation with the command, "ATTENTION TYPIST."  He was a real peach.  He had no friends in the transcription department, I can assure you.  Later on, when I graduated from college and was an RN he had the gall to call up the nursing station and demand that one of us "girls" get over across the street to his office as one of the patients we had cared for in the hospital the previous week had gone over to his office for an "office visit" (that was because he was so pompous that he did not wish to see that poor patient in the hospital). The patient while in his office  "defecated" on his carpet and he felt that was our "job" to clean up after incontinent patients even if they were in HIS office.   I swear this is true.  We told him he should call a cleaning service.  He blew a gasket.   I guess it could have been worse if we had the Chimpanzee guy.  I think I would have knocked him in the head after I sued him for harassment.  I saw down further where someone hoped he was a now a patient.  Me too and I hope he is constipated and no one gives him a stool softener.  I hope when he rides in the ambulance from the acute care to the rest home that he overhears the ambulance attendants refer to their call as a "lizard run."  I hope someone puts cornflakes in his bed.  I can't stand pomposity!  I hope he is roommates with "Attention Typist" - they might actually understand each other.

Just hang in there.  Pay special attention to every report, especially the errors you have done in the past, and follow your account specifics exactly (keep a copy next to you)  Practice makes perfect.

that I should use the term offshoring instead of outsourcing. We use the word outsourcing at my hospital so this didn't even occur to me. So just wanted to address this:) thanks again!

Has she ever actually dictated a sm
report in the format that she is required to do.  I do forms for a cardiologist in this manner, but when I first started working for him, he actually dictated the letters.  I think all physicians need to include certain things for a new patient, and then can do follow-up letters without including past medical history, allergies, etc.  I wonder if she has any colleagues that would be willing to share samples of their reports, but that specialize in the same field.
People, please pay attention. This isn't just one person
that had a personalty conflict of whatever.  Everyday there is someone else with a valid issue. 
no problems. you must learn to pay attention as you go along. once is enough.
btw, if it is a very difficult person to do i will listen twice in the course of transcription and then flag it for QA.

Attention QA people - PLEASE READ :)
I'm fed up with the company I work for.  Can any of you who are HAPPY with your QA job tell me what company you work for?  I have been looking but it's hard to know which companies offer :1) Decent pay;  2) A relatively flexible schedule; and, very important 3) Appreciation!  I would like to find a smaller company as I work for a very LARGE national company (can you guess who? lol) - I'm sick of feeling ignored and invisible and would like to work for a smaller to medium sized company that actually values its employees.  Please, don't say you can't tell me who you work for - I really need some guidance here!  Thanks.

No - don't give it up - just keep going. Pay attention to their style and what they want.
You will do fine. Stop beating yourself up. Every company and every account may have a different style they follow. You should have account specifics - just review those and follow their guidelines.
