Problem is - same problem there has always been!

Dew MDs bother to READ the reports or report any errors. Worked in medical records. MDs came in, sometimes by appointment!, and signed ALL charts at once, dozens at a time, sometimes color-coded tags for them to find the sig lines more easily! They would zip through, sign them, done. Then the chart gets filed away. And if they do look at them during the time patient is in-house for reference, they don't bother to report any errors! Why? I don't get it. I think every patient should get a copy of their hospital record and go over it thoroughly. I did and what a mess! Not only the transcription but the dictator screwed up on my meds and my history - he was apparently using old info from my initial consultation at the office and of course some of my meds had changed/stopped which I told him and he listed them on my H&P as current anyway! Also listed a history of depression when I only had one episode of anxiety around the time of my initial consultation. For the transcription part, it was an emergency admit - and the MT typed that I was asymptomatic instead of symptomatic! That caused a problem with my insurance being paid, of course. If I was asymptomatic, why was it an emergency? LOL. It's a scary medical world out there and apparently about to get even more scary!
