We do our calculating based on a scoring system. Each mistake has a point value assigned to it based on the severity of the mistake. This keeps a misplaced comma at a less point value than a misused medical term. We add up the point values, then divide them by the line count and that will give us our error rate. I am sure that other companies have different methods.
