 Yes, the trend is away from hyphenating words like reevaluation, intraabdominal, etc.  I don't care for it, but I would follow your QA's directions in this regard.
 
 Allergies are usually transcribed in all caps, sometimes in bold text, but it varies from account to account.
 
 Penicillin is a generic and should not be capitalized as a general rule, but follow instructions as regards allergies.
