You first would have to dedicate yourself to learning one of the machine ShortHand theories.  If you know nothing about writing in shorthand then the best route is to go through a program that teaches this.  They have several online schools out there both public and private.  (Green River Community College in Washington State has an excellent online program).  Really learn your theory well.  Then after you get through probably the 160 level maybe a good time to transition into doing medical reports.
