You owe this QA lady a big thanks.  These are all basic rules for good grammar and proper formatting.  If these rules have not been brought to your attention before now, it only emphasizes the poor state of basic education.  If the others are not being treated the same, consider yourself lucky that you are getting special treatment and being shown the correct way rather than allowing your work to go through and appear sloppy and as though it's done by an poorly educated person.  Informal notes (such as these postings) don't necessarily require perfection in terms of grammar, style, punctuation, etc., but a formal document such as those reports transcribed for physicians, DO require precision so, again, you're lucky this lady is taking the time to provide you with the proper education.
