Yes - but are you in a spanish-speaking country dictating medical reports and earning the salary of a physician? If you are going to a country, taking a high-paid professional position, then you had better know how to communicate in that language.

How many other countries let foreigners who can barely speak their language come in and practice medicine? None. Most countries have laws protecting their citizens' jobs from being taken away from immigrants, but not here.
