just plain wrong abbreviations. Afib is considerered slang at my company and slang isn't allowed no matter if dictated or not, verbatum or not.

Yes they are...I was just looking at a DVD burner called VerbatUm before i posted..
Never claimed we do not make mistakes and they we were not human HOWEVER, the mistakes I come across are really quite ridiculous when they could be caught by READING ones work over before submitting.
