 I leave a blank at anything I question and want to research.  When I come to the end of the report, I go back to my blanks and research them all at one time.  This way, I have a complete picture of the patient.  Also, I may hear the same word more than once and understand it.  This helps me tremendously.  Sometimes after the blank I will type what I "think" he is saying, to help me in my research.  It breaks up my flow to stop and research every blank.  Hope this helps.
