I have a rather complicated setup under my desk but it helps with the circulation, as well as the pain in the feet and heels which I have experienced.  First of all, I use a footrest, nice stable one that doesn't move around.  It's angled to be higher in the back and lower in the front.  It has like a carpet-type of material covering it, so it's not hard plastic on the top. 


On top of the footrest, to the right side, I set an old mouse pad/wrist rest combo thing so that the wrist rest part (high gel part) is at the bottom.  Then I put my foot pedal up on top of the part of the mouse pad/wrist rest combo part.  I also had an old long narrow wrist rest that I put down directly on the floor below the footrest, so that when I transcribe, I can either set my heel down on the long foot rest and just tap my toes on the foot pedal and the gel part is now in my arch, or I can set my heel down on the gel part of the wrist rest and just use my ankle muscles to activate the foot pedal.


I now it sounds complicated, but if you have any old gel/bean type of ergonomic supplies lying around, try them.  You might find something comfortable.
