Choose "physical therapy" from the specialty list, and then play around with the "type" list. I found a few sample PT reports under "SOAP notes."

Other than that... your best bet would probably be orthopedic note samples. The above is the only site I found in my admittedly-short list of sample report sites that included any physical therapy samples.

Does the client have any sample reports you can borrow?

And of course, you will save lots and lots of samples yourself, once you get started... right?? (If I had a dollar for every time one of my own saved samples has saved my bacon, I could retire.)
