I transcribe for an MTSO, and for the hospital account I work on, they actually want us to add the "we" to sentences. We are allowed to and encouraged to add articles (the, a, an) and pronouns (I, she, he, etc.)whenever missing from a sentence to make for easier reading. Heck, I don't mind... it helps boost my line count! But anyway, just wanted to mention that the rules will vary depending on a facility's preference.
