When I was on a large ER account using IT I did make individual glossaries for each physician (about 30-35 I believe). It seems like a lot but I was able to get fabulous compilations for their glossaries. I think it would depend on if you use large templates already for them or not. If you already have a template for each one with all of the PEs, etc., I would probably stick with the 1 glossary. If not I'd go with indivdiual glossaries, save all reports, and get some great compilations on each doc.
