"Do not use commas to seperate drug names from doses and instructions. In a series of drugs for each of which the dose and/or instructions are given, use commas to separate each complete entry. Exceptions: Use semicolons or periods when entries in the series have internal commas. Medications may also be listed vertically."
I.e.
The patient was discharged on Carafate 1 g four times daily, 40 minutes after meals and at bedtime; bethanechol 25 mg p.o. q.i.d.; and Reglan 5 mg at bedtime on a trial basis."

BTW-I got my BOS off ebay brand new, saved me about $40.
