I also had a lot of trouble and so I will often use three letters that I would remember and an "x" after them.   Since I also type for the same docs --I am lucky - in auto correct I will often put the same two to three letters and then a  (dot). and the docs initials.  That way I don't have to remember so many different abbreviations. i.e. hl.jn  hl..mb  hl.eif etc.  That is for their heart and lung exams.   I make it as simple as possibe for me as if I have to think too hard on what I named word in the abbreviation it is time loss for me.   Some docs I can type their exam with using about 20 Keystrokes for about 10 lines.  That is where I make my money.
