Katrina is going to impact us greatly in the wallets.
Prices everywhere are going to rise.  I am not an expert but I can almost feel everyone preparing to raise rates for services and products.  My biggest fear is insurance premiums.  Can anyone argue me out of this thought process?

You are right, Jenn. Most doctors do not want VR, they don't want to take the time to train on VR, and they don't want to have to correct their reports. They value a human MT.

It's the almighty dollar that has the last word. It's expensive to implement, but cheaper in the long run to use VR rather than human MTs. Administration is not concerned with the quality, only the expense. If it looks good on paper, it's a deal. VR is bundled with PACS in radiology settings - and all radiology facilities will eventually be using PACS as x-ray film has become obsolete. A digitized radiology dept. does not have to worry about huge files to store, processing equipment, darkrooms and developing chemicals/waste.

Teleradiology is here and medical facilities must keep up with the technology to provide their services. Rad MT positions will be hard to come by as facilities make the switch to digitized systems. For rad MTs, it isn't looking good down the road.
There can also be placental and cord abnormalities that impact things sm
I have transcribed a 2-vessel cord, though they should be 3 vessels. I have had velamentous cord where the Wharton's jelly didn't cover the cord structures up near the placenta which can be dangerous to the baby if the placenta is located in certain places. If it doesn't come on its own, it may be retained placenta (treated with D&C) or accreta meaning it has grown into the myometrium, necessitating a hysterectomy and fast so that the mother lives. You also have true knots in cords (my younger son had one) and you won't see that until the placenta is delivered.

There are 3 stages of childbirth. They are laboring, then active, which is pushing and then delivery of the placenta.

Not a silly question by any means. The placenta doesn't matter to most of us who have had kids, it is the only throw-away organ in humans. It is, however, important to the doctor.

Actually...more likely the almighty insurance companies or to take it even further, our almighty govt. is to blame.

MTSO's in order to compete with offshore and with doctors cutting back and insurance companies cutting back, have had to lower what they charge doctors (or lose business to offshore), therefore they have to pay MTs less. It's a vicious cycle. It's happening in the tech industry, as well.

If we don't like it, we should think about how we VOTE, because that's the only power we have left at this point.

I totally agree with Patti. Who the heck do they think they are (AHDI)to change everything they feel like on a whim. It's all about the money for them and we all know that.
Yes,, and this has a huge impact on quality when people get into strange accounts but it keeps QA
busy and ties the work up there then. You just cant put tons of people into unfamiliar accounts and think the quality will be there not to mention the time it takes to do new accounts so you are losing money big time but I guess it is better than nothing. Much better to have familiar accounts for both quality and line capability.

About loyalty to MQ, sorry lm

Unfortunately, MedQuist has been made who they are by buying out so many small & medium MTSO's over the years to get where they are, and I wonder who was not thinking about a consistent "MQ way" while they were doing so.  I mean, as much as we refer to McD's and Wal-Mart, when one opens they sell the same/serve the same around the country.  MQ was like a game of Pac-man, gobbeling in hyperspeed.


In doing so, they made deals with the sales to keep things "as is" it seems from some of the posts and the vast inconsitency across the board has led to so many, many different ways of operations, how MT's have/were treated, how offices are ran, mass exodus of inhouse employee's.  We see this in the praises of some CSC's with plenty of work, treat their MT's with respect, then of course, the nightmare CSC's.  This total opposit just does not make sense at all.  If some need work, then give them work from an office that is overwhelmed for crap's sake...spread the work and $$ amonst all.  I don't have a big suit in my closet or a big degree hanging on my wall, but it sure the heck seems like MQ could save $$100,000' in just "consulting with the MT's" in how work flow could be consistent, accurate and done with pride, care and loyalty, instead of paying those high priced suits that seem to come and go like a revolving door.  In their quest to be the biggest and best with the hypertechnology, they have forgotten what it's really all about...the patient and the MT who is taking pride and time in doing that record because the MT is not breaking their backs working 12 hrs to make what they did in 6, they know the account, they know the dictator and that does not leave them guessing.  I know when one of my doc's says something really off the wall, out of character and does not make sense...as in being interupted when he's giving me drug allergies, says NKDA yet gives the patient Cipro because of a PCN allergy...that's a discrepancy that needs to be sent to QA, so that the next person or someone reading that record does not just glance see nkda's.  I got a very nice thank you from the doc on this and it's only because I know he was very annoyed at whoever was bugging him, and this is because I have him night after night, year after year.  That's what happens when you have accounts you are proficient at...they get done in quality and faster!


This is how you can have so many people on either one side of the fence or the other when it comes to MQ..and there really does'nt seem to be much in the middle.  If MQ has treated someone decent and fair, then it's easy to be loyal to them and vice versa.  There's just never one side to a teeter totter and we may see a huge imbalance one way or the other with the anticipated packets...........I just wish they would get here and calm a few nerves!

I worked for the same two groups of physicians for many years and when they were able to cut me out, EMR or make office staff transcribe for less, they did. It's all about the $$$. I never was able to get any of them to sign a contract other than one that stated that they could let me go at any time or I could let them go at any time. Of course, I would have been more considerate and gave them notice and been honest about it. Even at cut rates, they are always looking for a better deal. One of the office staff who took my transcription work on the side at cheaper rates recently called me and said she needed help, etc. but at her lower undercut rate.. (what a joke) I at least had the satisfaction of telling her "No thanks". She said she did not know it would be this "time consuming" she has no life, yadda, yadda... They thought it was soooo easy...NOT! Sorry about your situation.. it's a tough lesson we all eventually learn.

Like you, I worked as an employee for a group of 18 physicians for over 21 years. I retired early, then they begged me to be a contingent employee because they had no one to fill in for vacations, sick leave, etc. I agreed and it was a pretty good deal. They paid me as a regular employee, albeit contingent basis only, which was fine with me. If I wanted to work, I did; if I didn't or couldn't, I didn't! Then a year ago they switched to a new Dictaphone call in system (whereas before we transcribed from tapes). I said, "There goes my little part-time job." Oh no, they wanted me to stay, blah, blah, blah. Bottom line: They got the new equipment and didn't even have the decency to let me know they weren't EVER going to use me again! Over the years, I paid for much of my own equipment working at home for them and had been their ONLY Transcriptionist for over eight years (with 8 docs at the time!)until the practice got too large. In fact, I had more experience than all of the transcriptionists there! Several of my coworkers thought it was pretty sad but they couldn't do anything about it, of course.
Again, money talks and since I was most probably the highest paid transcriptionist there, they wanted to dump me to help pay for their leased equipment, I guess. Finally when I asked the lead transcriptionist about all of this, she said they weren't going to use contingent people any longer. So my opinion of docs has definitely changed during the last year...and NOT for the better. Sorry for the long post but I get angry every time I think about what they did.

I know summer is usually a slow time in the field of medical transcription but I have sat for many days now with absolutely no work.  The past two pay periods have been extremely rough.  I like the company I work for but I can't afford to sit idle.  I have started looking around and have even sent out a few resumes.  I feel a bit disloyal but I have to eat too.  I find it hard to believe that there is absolutely nothing! 


 


Do you ever feel anxiety when there is no work, and if so, how do you deal with it?  Am I being disloyal by looking for work?

It was wonderful, especially since I was single Mom and just lost a supervisor transcription position at a hospital that closed. I was lucky and landed another job immediately with a private clinic, but was able to take my daughter on a few trips with this extra money and pay off a few bills.  I feel really fortunate now because I just remarried a year ago and money is definitely more stable, but i still hate being treated dirty when I was so loyal to this clinic. 


Is $1000 to much up front????  There really wont' be that much typing involved, but my time in finding all this information on my own was a pain butt!

And third comment.  ISSUES!  Just because this person is American and lost her daughter does not give her the right to call everyone but herself incompetent.  She CANNOT call these people criminal w/o EVIDENCE.  I wish you would think that thru.  If she is so SURE these people are criminals, let her step up to the plate with EVIDENCE and share that with the world she is urging not to take in the two brothers who were released for LACK OF EVIDENCE.  So tell us what EVIDENCE she has to stand in judgement of everyone.   Oh that's right.  She is American and lost her daughter.  We don't expect evidence just rambling emotions based on.....????
You owe them no loyalty, especially if they don't pay squat.
Send them a note thanking them for the wonderful experience but giving your two weeks notice or whatever because you have accepted employment elsewhere.

How loyal is the company to you if they pay you peanuts, and how hard would they cry if they decided to "down size" and let you go?
IC loyalty dilemma

The owner of the company I contract through has a strict policy that you do NOT ever converse directly with the client.  I am on an account where there are 4 of us doing it.  One girl continuously keeps writing the client with everything from asking about formatting to asking what some medical terms are.  I have asked her in a prevous e-mail to please not do that, due to the work policy.  She has continued to do so.  I don't know what to do.  If I turn her in, she would know it came from me, and would I be a rat?  She claims that she and the client made an agreement to e-mail each other without telling the owner of the MT company.

And be honest. This is a financial issue and a matter of there is only so much time in one day. They may be shook up for a bit but in the end, they will remember you for the work you produced and your day to day performance and personal character traits. Give them plenty of time to find someone new if you are very fond of them and, in this case, you could offer to train the new person.

I'm amazed what i can find there for $1. You never know what you're going to find, but cleaning supplies, household stuff, things that are definitely more than $1 even at Wal-Mart...as long as you're willing to not be picky. When the hurricanes were looming, I got mutli pack of name -brand batteries for $1, tons of candles, etc. You just have to search the whole store, but definitely worth it.

Trying to stay around not more than 275,000.  Would like 3 bedrooms, and 2 to 2.5 baths with an office or fourth bedroom for work.  A pool would be nice.  When we were there about three weeks ago all the new tracts were on lotteries or about a one year wait to build.  Don't really wanna hassle with the lottery since I don't live local.  How about Palm Valley?  Is that a nice place?  What would you say is the community with the best schools.  My priority is location, schools, price, community amenities, not too far from everything so hubby does not have a monstrous commute, etc.

If Canadian MTs are making more $$ than American MTs, it all evens out.  Also, the national health care system in Canada is currently under revision and my cousin tells me it takes her 6 months to get an appointment for an XRAY in Calgary!!!........


Canada's dollar is not as strong as our dollar in the US and I LOVE CANADA so please do not think I'm bashing that wonderful sister country of ours.

 line expectations/requirements up to 1800 a day, the QA/editors I saw expectations 3000 a day, 2,3,4 cents a line. 


What do you suppose this comes out to hourly, say 4 pages an hour, 35 lines/page times 140 lines an hour, say 6,7,8,9. And, now what are the requirements.


.I see where they expect/require the MTs to pay for just everything, THEIR costs to do business.  Recently saw -- need 3 phone lines, provide at your cost, long distance plan buy this and that. Nonsense. .


After all this output who knows if in 30 days either side wants to continue relationship and look at all the money and equipment, Lanier $250 footpedal, thousands on some type of Lanier equipment LX something, 500 deposit to return in 6 months, this is nothing more than free interest loan for the business to fund their equipment purchase. 


There is a lot of anger, frustration and disrespect out there, you can tell by the ads posted, a slave-like attitude toward the MTs.


Frustration/anger fosters errors.


AND, look at what the MTs have to put up with in terms of dictation, if we can't hear how to make production and accuracy. It is, anything goes, take a deep gulp and see if you can finish report before you need to take another breath.  Dictate at live football games and "whisper" Intentional speed dictation because they don't want to say it again, no matter how many times yuu rewind, can't make out the dictation.  Anything goes, just dictate dear doctor, just dictate, we expect/require 99.5% accuracy.


Yep, I too am frustrated and saddened.


What could be done is--- nicely state on ads, be sure and run a spell check, "smile"---- problem solved ----instead of the disrespect you feel..

I think they are just being pains.  There are some people they bend over backwards for, some they don't work with at all and unfortunately I am at the lower end of the stick, it seems.   I had a coworker quit and went to work for an on-line service because they would not let her have Sundays off so she could go to church with her family even though they had adequate coverage on Sunday and by Thursday we were always out of work. 



The dollar amount may not have been the point...
but I just find it funny that those who agree all say they are making $25+ an hour. Just wondered if you would still agree with her if you weren't making the money you are making. Many of us do support our family on our wage and there is no way I could do it on $11.50. I make a few dollars more an hour than that and we are still struggling to make ends meet. I guess I was just put off by the judgmental attitude of the OP that if we can't cut it with our current pay that we just need to reevaluate our lifestyle. I live simply, but I live in a high-cost area. There is really no way I could cut back on my costs. So if I ever make less than I am now, I will be forced to look for a new job.

The position varies from place to place..
Sometimes you end up doing VR, which can be really easy or a real pain. Sometimes you end up simply editing files and all is good. Other times, you end up with more job responsibilities and hours than you ever wanted. All in all better than typing LOL.
Dollar store reading glasses work for me.

Just bought 4 pair at a 2.25 magnification today, as the more I do this, the worse it gets.  Started with a 1.25 a year ago.


I knew it was time when I had to wear two pair of 1.75's on top of each other so I could thread my sewing machine needle.

We just signed up for our propane for winter.. It's gone up a dollar per gallon...
from last year.  There is no shortage of propane as far as I know.  They just raise the price because they can.  We're paying 1.89 per gallon.  Thank goodness we have 2 wood stoves, but I'm sure a cord of wood will double too. . You can't win.
I have a sister who we call the "Queen of the Dollar Stores."
Bless her heart, she buys all her Christmas at dollar stores (decorations and all) and even gives me makeup or hair products that she has tried and doesn't like. She doesn't know that I know this, though. I just smile, hug her and go on. It's not that she can't afford any better, she just sweeps the dollar stores, gets everyone's gifts in about 30 minutes, and she is through for the season. I guess it's not supposed to be about the presents, but the thought (but sometimes I wonder what that thought was, LOL)!  MERRY CHRISTMAS!

I have been at this 25 years, even once had my own accounts and worked 24/7 with cat naps to keep going but no real sleep. I really burned myself out. Now I work a regular 40-hour week, but it is not my life. It is a means of making money so I can go live my life. I work for a company that is not all about production. They want us to take our time and do it right. I am doing that, and it feels so good to be able to work at my craft and be allowed to do a good job while I am at work.

But after I get off, I have my family and I take time to have fun and enjoy life. I think that is the key to not staying burned out. You need down time, time to have fun, exercise, relax, etc.

Exercise is very important, too. It rejuvenates the hormone system so you have more positive hormones flowing through your body and helps get rid of stress.

That's what I do.
