ergonomic questions

The pain was in the lower part my arm. It has subsided over the last couple of days. I TRULY appreciate all these suggestions. Along with this pain, I have also had the tingling in my wrists on occasion. I absolutely love my new job but I know my health has to come first. I will check out all your suggestions. Thanks!

I just recently went back to work in the office at a local hospital. We type in both Meditech and Cerner, depending on what you are typing, i.e. Medical Records reports versus Radiology.  We are currently typing blindly in Meditech, meaning we do not have access to our line counts and must depend on management to give us our totals.  In Cerner, we are able to run a line count report and print out a hard copy of our line counts at the end of the day.

We are finding that 800 lines in Cerner equals about 500 lines in Meditech.  We are not paid per line nor do we have an incentive plan, but we have a new HIM Director who would like to institute an incentive plan and the administrator over our department has laid down the law and decreed that all transcriptionists not making minimum in the department will be written up.  Problem is according to Meditech line totals that we have been given, that would be about half the department.

The transcription supervisor has taken our concerns to the new director who told the MT Supervisor to hold off on writing people and has taken home all the line counts from the last month and she is going over them with a fine tooth comb this weekend.

I am not a very trusting soul by nature, so I am doing a little research on my own.  I've been in this business for about 15 years.  I started out in a hospital and have worked for a few services at home and am now back in the hospital setting.  So I know a few things about line count and I consider myself to be pretty well informed on trends and technology in the industry.   I would like to walk into our next department meeting armed with as much knowledge as possible.  So I have come up with some questions that I would like some fellow MTs to answer, give opinions, and maybe even direct me to websites or individuals who could give me a better understanding of Meditech and Cerner.  Here are my questions:

 


  1. Is it possible for an MT to have access to her line count in Meditech or is strictly a management function?  Any MTs that work in Meditech on this board, do you currently have access to your line counts and if so, how do you check them?  Is it an option on the main menu after you log into Meditech?

  2. How is a line defined in Meditech?  (i.e. 65 cpl, with spaces, without spaces)

  3. How is a line defined in Cerner?

  4. What abbreviation Expanders work with both Meditech and Cerner?

  5. Examples of incentive plans (if anyone would like to share that with me that would be great, if not I completely understand).

  6. Measuring productivity by minutes of dictation versus typed lines.


    1. What would be the minimum minutes required for an 8-hour day?

    2. How would one structure an incentive program based on minutes?

I was thinking of presenting the idea that we keep track of productivity using minutes instead of lines since there is such a discrepancy in Meditech.  I also worry that if an incentive plan is put into place and we are still typing blind into Meditech, what's to keep management from shaving off a few lines here and there?

if you would take the time to adjust, the Kinesis Classic is great. AND you can reprogram keys. just the difference in location of keys and key-depth has already helped my CTS a lot!

Not only is it impossible to use, the salesperson was  rude and difficult when I called for return instructions.  Also, there was a money back guarantee on the website, but in small print "if refurbished, we cannot refund your money."  And guess what, you don't know its refurbished until it arrives on your doorstep!!  Needless to say, 350 bucks flushed down the toilet. 


I recently ordered the "ergoflex" keyboard, and was wondering if anyone has tried this?  It lets you rest each arm as far away as possible, which is what I need.  I've been typing on two keyboards, one for the left, one for the right, and it just takes up too much space.  I'm hoping this will help.  To the poster below about keyboard testimonials, these could probably be secured to the end of your arm rests, and also on the web site they have separate arm rests that look real comfy you can adjust to your table top.

Ergonomic question
I have been workign as an MT for only a couple of months now, but I already am having pain in my arm.  It started being sore yesterday and continues today.  Is there anything I can do to help ease this discomfort and should I invest in an ergonomic keyboard.  Thanks!

YES, YES, YES!!!
I purchased a refurbished SafeType keyboard because I started having tingling, numb wrists and hands and the difference.....WOW!! I told every MT I knew about the difference they make.....WHAT A GREAT INVESTMENT THAT WAS!!
I could tell the FIRST day of using it (after I got used to it) that my pain and tingling was GONE!! The keys are sideways and I thought it would take me forever to get used to it, but it didn't! I thought it was worth a try as a last resort in "ergonomic keyboards." I was using a split keyboard and I got used to the SafeType faster than I got used to the split keyboard when I started using it. I'm actually faster with the SafeType. They retail for around $300, but they sell refurbished ones for around half that. IT TRULY WAS THE BEST INVESTMENT I EVER MADE FOR TRANSCRIPTION!! If you haven't heard of them, check them out.... www.safetype.com, I think. THE MOUSE is GREAT, too! It's only around $80 new, but I lucked into getting a demo for about half that too!

ergonomic advice
Anyone know of good websites or where to get advice about setting up your home office ergonomically correct. Sometimes I think I have my chair set at the right height, or everything just so, and try to stretch every once in a while, but I still end up stiff and I think maybe I could do better with it. Thanks all.

Ergonomic Keyboard
I have always used a split keyboard, however, just purchased the Microsoft 4000 and have found it a little difficult to adjust to. The keys and space bar are hard to press and I am running words together because I think they space but don't. Other than that I love the split style and could never use a regular keyboard again.

Ergonomic mouse?
I have a Microsoft split keyboard, that helps my wrists, but I'm noticing that when I use my mouse a lot (which sometimes is half of my day), my elbow and forearm are getting sore.

Has anyone had this problem with a mouse (looked it up, and it's called "mouse elbow" similar to tennis elbow). If so, what was/is your solution? Looking at new keyboard with a built in keypad, and/or a different mouse or something, and wondered what anyone else has done.

MS ergonomic keyboard
help!  I have the MS ergonomics 4000 keyboard.  My F7 key would not work at all yesterday and  now the F5 and F9 will not work which I need to use for the company that I work for.  my tech dept. did not seem to know what the heck is going on.  Any suggestions welcomed!  Thank you,
ERGONOMIC KEYBOARD

Probably about 10 or more years ago, started having same problem.  Switched to ergonomic keyboard, and have been using ever since.  I have a Microsoft, I think it is Microsoft Elite, or something like that.  There is also a Microsoft 2000 available.  Just check on the Internet for availability at discount computer stores, or try E-Bay. 


Would definitely recommend!


Ergonomic Keyboard

I have been using a Microsoft keyboard that seems like forever (since 2002).  It has survived spills and scrubbing when I clean it.  The only problem is that I have worn off some of the letters on the keys.  It has its advantages - keeps my son from using my computer because he is not that familiar with the keys.


 


Are you using an ergonomic keyboard, that may help some.

I used to use a DataHand, but they are out of production now and I'm so disappointed. What is everyone else using?
Ergonomic Chair
A good chair is a MUST.  If you're using one of your dining chairs, you're neck and lower back are going to start giving you painful lectures!  I got my task chair years ago from Office Max - adjustable tilt seat, adjustable height back rest, height of chair itself adjusts of course.  Best $200 I ever spent.  You can probably find something similar that is used and reasonably priced in your neighborhood Craigslist listings. Good luck!

what part of the hand? I get pain in my wrists and wear wrist support, but I also know my ergonomic keyboard makes a big difference. I would say stretched, but not sure of what hand movements you could do, cause like i said mine is the wrist.
oh and anti-inflammatory round-the-clock. hope it gets better, it's not pleasant to work through :(

Goldtouch ergonomic keyboard
Check out ebay for keyboards.  I use a Goldtouch, whic tents up in the middle, like a teepee.  Got used to it real quick.  Now it seems totally strange and uncomfortable to use a "regular" keyboard.

Ergonomic Keyboard - My Experience
I use the Microsoft Natural Multimedia Keyboard which is a split ergonomic style and I think it is great, but I had to get used to it by using it a lot and I could not resort to using the other keyboard while getting used to it, otherwise it made it more difficult. Once I got used to it, I was fine. Now if I have to type on a regular keyboard, the keys now seem too close together. I also find that keeping my keyboard (either one) low near my lap and my hands at a neutral extension so that my wrists are not flexed either up or down has kept me from incurring any injuries or syndromes (CTS) connected with our type of work. I bought a tray for keyboard that I can adjust in this way. I have been a paralegal in the past and I am currently an MT culminating in over 20 years of typing and my hands and wrists are still fine.
I have a split ergonomic keyboard
and LOVE it.  It took a few days to get used to it, but now I don't know how I typed all those years on a straight keyboard.  This is MUCH better.

4000 Ergonomic Keyboard
I just purchased and using the Microsoft 4000 ergonomic keyboard and love it.  It took a couple of days to get used to the keys.  I have used other ergonomic keyboards, but this one is the best.

MS Ergonomic Natural 4000...
I admit, it took me a few days to get used to where the keys are, but I bought it for my wrists.  Two things:  My wrists have quit burning and the keys are very soft touch.  I love it.  I got mine at an office store for 59 dollars.  I guess not cheap, but it was worth it for me since I plan to type until my little fingers won't work anymore.  
Microsoft 4400 Ergonomic- sm-
I like it alot because of the raised wrist rest (removable), which helps with wrist pain after typing 10 to 12 hours a day. It has been a blessing to me, but getting used to the split keyboard took about a week. Check it out.
MIcrosoft Natural Ergonomic

Best one I ever had.  It has all the programmable shortcut buttons at the top for programs and/or webpages and is extremely comfortable.  I used to have problems with my hands hurting a lot after a full day of working and now it is gone.  It took about a week for me to get used to the feel of the split board, but now I can't stand using a regular one.  I even take this one when I travel to my in-laws.

Ergonomic Keyboards Rock!!
I absolutely cannot type using a regular keyboard anymore. I freaked out last year when mine croaked and I couldn't find the same model I use at Staples. I finally found one at Amazon and had them send it to me ASAP. IF I ever get any extra money I might order a couple more to keep on hand in case of emergency.

and have tried most of them, Goldtouch and Kinesis (2 types) and a very expensive one that split in the middle and tented up or down but can't remember the name of that one. All of them were a vast improvement over the regular keyboard and helped my carpal tunnel significantly. I am currently using the Microsoft 4000 and would highly recommend it. I don't think you would lose any significant production time at all, very easy to adapt to. Good luck.
Instructions for a DIY ergonomic keyboard.
I admit, I an avid nut for creative problem-solving. As part of that addiction, I am a coach for a creative problem-solving team of kids in the DestinationImagination program. Through that involvement, I came to love the Instructables site. Today, there I came across instructions for an ergonomic keyboard and wanted to share them with those of you out there who are handy or married to a handy spouse. Let me know if anyone takes this project on and how it turns out.
Anyone using the Quill ergonomic mouse?

I need to try this but would like opinions first.  I have developed trigger finger of my middle digit on my right hand from all the scrolling I do with the mouse.  I only have 1 more injection before surgery is considered and I would REALLY like to avoid it if possible.

I have a Microsoft Natural Ergonomic.
I got it on sale at Staples for around $60. When I used to work in a hospital, I had one for home and bought one to use at the hospital. They are not all that expensive and well worth it.

all the way.  I also use the Microsoft one.  I started using it about 6 or 7 years ago.  Funny thing though, once you start it's really hard to go back to a regular keyboard as it seems so tiny and crammed.

just purchased a store-return new backup keyboard through Amazon for around $25 dollars ..


New, at my local CompUSA they were close to $60 regularly and went down to around $40 on special ... that store closed and I'm not sure where I would buy one now (most places do not carry them) ...the time and angst spent replacing a keyboard in a hurry is REAL and well worth preparing for (also keyboards keep getting  redesigned which is really annoying when you're typing all day and have to switch in a hurry -- so it's great to have an "old model" on standby -- I last had to replace my keyboard when I popped off a key that was sticking and it went flying and I have NEVER found it ... it was an N) 


I've found this particular model to be comfy and durable --- I didn't think I would like it and I really do -- it's quiet and has good key play.


My next purchase is going to be QuickLook Drug book on DISK since it can do wildcard searches of partial lookup -- my old web site for that "changed" and is not helpful and there are SO many bad Google entries.


Anything that makes you more comfortable with less fighting your desk also helps --- a decent chair and glare protection, a foot rest .


and SAMPLES when I start a new account I save almost every report I type, even if they are full of blanks -- which I fill in when I review my QA  -- invaluable for saving time figuring out H&P mumbles and  particularly op note


I use RED NOTEBOOK from Horus development (link below) ... I am not aware of anything comparable ... it's searchable. You can, for instance, upload a hospital medical staff roster  if saved in TXT, edit and print out lists,  just fabulous for new accounts with new terms / medications/ names for things.

Does anyone use of those DataHand Ergonomic keyboards?....
I'd love to hear opinions on this.  It looks interesting but is there a learning curve...do you have to learn a whole new layout?  I've got to try something different as my hands/wrists/arms are killing me and I think I may be going down the carpal tunnel road.  I do mostly voice recognition editing and wonder if anyone has had a good experience using this with VR as well as regular transcription.  Please, any comments or opinions are very much appreciated.

My sentiments go out to you as I too feel that most foot pedals need some rejuvenation of some sort as I have had pain too. Sometimes I switch from one foot to the other throughout my shift and sometimes I even stand for a few hours while transcribing to get my blood circulating.


A lighter touch foot pedal (more sensitive) would take some getting used to but definitely might be a good thing to get used to. Just like the old keyboards you had to pound were bad for the hands and the newer ones have a much lighter touch and are quieter. Just bought a new logitech keyboard at Walmart (wired) and only $11 and was amazed at the nice quality of it and the touch is like butter.


Well, I know you didn't want to know about keyboards but just though I would share in case you were looking for one of those also.


  


Anyone out there use ergonomic split keyboard?
I bought a micrsoft ergo split keyboard. I am not sure if I am going to like this. Apparently one is supposed to use their left hand for the letter B....after all these years I have been using my right! Whoops. Anyone out there really see or feel the difference in the comfort or have any of you tried it for a long time and still just do not like ergos? Let me know your thoughts.................thanks friends!

Kinesis Contoured Ergonomic Keyboard

Has anyone used this keyboard?  (It is the weird shaped one).  How comfortable are the hard plastic wrist rests?


Anyone use the Microsoft Natural Ergonomic 4000
keyboard?  Lately my wrists have been hurting and I've been having to take too many breaks.  Of course, can't afford to do that.  I was hoping if someone has the Microsoft Natural Ergonomic 4000 keyboard they could give advice on whether they think it helps their wrists as compared to a regular keyboard. 

 

I can't afford to buy it if it doesn't work YET I can't afford the loss in production from this wrist pain.  Funny, it's always been my ankles that got sore, now it's my wrists instead. 

 

 They do have the keyboard above on sale at Best Buy right now for only $50, but even that is too much IF it doesn't help with the wrist pain.
