here is what I do

when on a new account, first I concentrate on getting the system down while at the same time acclamating to the doctors. (I work with small goups so get the same ones alot). I find what works for me regarding pace of getting in and out of a report.

Once navigating starts to become second nature I pay more attention to doctors and catching repeat phrases.

On shortcuts, I start with generic phrases, headings, typo's (het,the), words I simply hate typing (weak pinkies), words I can never remember how to spell, and any and all drugs I come across that I cannot remember if capitalized or not. and so on.

Also, after a while I start to hear and realize which doctor says what and how often, adding doctor-specific lines or templates.

the more I get used to doing the account, the more moves go on autopilot.

The speed picks up from navigation on down to autopilot. Once you are so comfortable you are bored, and accuracy is not an issue, pick it up a notch till comfy, and repeat.
