You really have to give it time. How is your typing speed? It does speed up considerably as time goes by. Also, I agree with the "Advice" poster below about working on your expander, including not just medical words such as... however, although, underwent, etc. Include entire phrases that are used constantly, i.e. *The patient is*, headers (I use a prefix of h plus abbreviation to differentiate it from a regular word), medications (especially the capitalized ones so that you can type it in lower case). Put entire reports in the Expander and, when not on duty, go back and remove patient-specific (age, medications, etc), thus leaving a *shell* for the next time.

Also, I had a mental block for the longest time with certain words like peroneal/perineal, so I made my abbreviation for perineal a short, socially unacceptable word for the between-the-legs area (which I won't type here because it would definitely offend some people), and haven't had a problem with it since! In other words, use the expander as a reminder tool for difficult words.
