Companies can't tell you what to do when you're not working for them, employee or IC. It's no different than holding down two different jobs somewhere else, as long as you work your assigned hours.
They usually have a "do not compete" clause in their contracts, but if you are already employed at a clinic that they service, let them know. If not, it's your time!!
