"She had been complaining at that time of 3 weeks of diahrrhea, which was watery, 1 episode per day."

The commas setting off "which was watery" are a given.

If your company wants you to follow BOS:

(new recommendation in BOS 2nd edition, Page 11, ... use numerals throughout ... to "facilitate prompt communication ..."
