I've been doing this 30+ years. Every time i have some kind of pain start, i work at alleviating it and still have no major problems. I did go to the kinesis keyboard -- would not have anything else now. When my foot hurts, i put something soft and cushy under my heel. When arms/wrist bothers me, sometimes i move my keyboard into my lap. Sometimes put it at an angle or move my chair up or down, etc. I'd say do not ignore pain or medicate it, but find body mechanics/ergonomics that work.
