Why would you write someone up
gee that is very encouraging, I have been disciplined even with my current job when I was in the mentor program but not written up !!!!! That style of company is not for me, I should have listened to the posts of how they are sweet and than watch out... I'm sure a lot of people are happy with them they seem to have a lot of employees
I just had to write this....
I am now one week free from my best friend is what I like to call smoking.. This was difficult to give up and for the first time in my life I sat in a nonsmoking section at a restaurant yesterday, I can run a mile, I am alert and awake and feel good, my skin is actually looking younger.  I have attempted to quite many many times... The conclusion to this was its my body, I control my body, cigarettes do not control me, I control me, God gives us one body enjoy it, take care of it, REMEMBER ITS YOUR BODY... Sorry if this offends anyone I just needed to voice myself for I feel free for the first time in my Life....

Synthroid 25 mg --hello? I have to change that one all the time. Or how about clonidine for anxiety? Or one of my personal faves respiratory rate 70, heart rate 16...happens all the time. Ladies give yourselves a pat on the back for fixing and "spotting" all of THEIR mistakes!
to write or not to write
I have always put on my resumes that I have the ability to transcribe foreign accented physicians and never had any comment made about it, so guess it worked for me.  The QA part, would state as you suggested, experienced in quality assurance....would look oh so pretty on the resume by all means.  Good luck.
You can write off anything you want to, you just have to
be able to document it if you get audited.  I would have DSL if I didn't need it for work and my whole family uses the internet so I can't in good faith write it off.  Even if I wouldn't have it if not needed for work I'm sure you still use it for personal use. 
Why you should write it down
The purpose to keeping a log of everything you do that is not actual transcription, i.e., is not typing to generate lines, is to show you how much time you spend not-working.

This can be a considerable amount of time. It's important to get a handle on it, though, because it goes a long way toward showing you where you can become more efficient.

Awareness of the "not-work" time -- I hesitate to call it wasted, because some of it is purposeful, even if it doesn't generate income -- is necessary if you are to control it.

Nearly everybody pops up out of the seat if it is a difficult dictator. When I code something difficult, I jump right up and find all sorts of errands to run. It's much easier to stay in the seat and stay focused if you are aware that you keep leaping up to run around.

Even taking a sip of coffee takes a few seconds. Looking away to the TV takes a split second here and there. Answering the phone takes a couple more. Pretty soon it's a whole minute, then an hour. If you have one hour a day in non-income-generating activities, if you made $10 per hour, you'd lose $10 a day, $50 a week, $200 a month, $2400 a year. Double that if you made $20 per hour.

Tax write off

Anyone know if we can write off our losses in the stock market last year.  I have no clue as I have always made money.  I sure my accountant is busy or I would call him.  Just curious, as my hubby took a big hit, should have listened to the bunionmeister.    Listen next time hubby.

Yes, as mother's we should take the
responsibility. Whether they were married or not is not relevant. Everyone makes poor choices in life. Everyone, married and unmarried, might need help at some point in their life. What difference would it make if my sister in law was married? The courts wouldn't enforce the order anymore than they are now. You say you are all for going after the father, what difference does it make then? They are not held accountable by your government, but you think it is okay to speak against any woman needing help only momentarily because they made a poor choice in life?

I'll give you another example. My mother who was married to my father for 10 YEARS had to get a divorce and take all 4 of us kids with her. My father after 5 years of MARRIAGE started beating her. He then put guns to my brother and my mom. He raped my sister many times before she was ever not scared enough to tell someone. She left as soon as she could get out and took all of us with her. Our government that she paid taxes into her whole life did not once honor the restraining orders, did not honor the court order for child support, and would not provide my sister with counseling at no charge or reduced rate. Do you know why? Because a member of the family committed the crime. Even though the man told my mom that he is aware that most all sexual assault cases occur in the family, but according to the procedures laid out to them by our government, you cannot get assistance if the act occurs in the family. My mother worked 3 jobs. We had to get her out of bed with a broom because she would come up swinging. She drove us all many, many times to counseling and did not get sleep to go to her next job. If our government would have pursued the child support order, if they would have offered her free counseling for my sister so she was not having to pay out of pocket, just a little help for a short period in her life, then she would not have had it so rough, or us so rough during those times. I am sorry, but it is our responsibility to take care of ourselves. On the other hand, our dollars do not go back to our people in the right way. If you think they do or you think that the government should not be held accountable for their lack of assistance or overseeing of the assistance they do give to make sure it is used properly, you are fooling yourself. If you think it is about all for yourself, never have to depend on the government at all in your life for anything, then give us our money back and we surely could do that.






Mother's Day
My birthday is on Mother's Day this year. I got myself a present, a ticket to see Rob Thomas in concert in PA. I usually get a combined birthday present/Mother's Day present from my husband and two daughters.
My mother's day
First thing when she got up, my 5-year-old asked "Is it Mother's Day?"  When I said yes, she went to her school backpack, pulled out a little brown bag that says "I love mom" on it, and took out a little sculpture of herself that she made at school. My 18-year-old son spent half the day sleeping, a few hours at his girlfriend's house, stopped at home and gave me the top half of a carnation. I think he pulled it off of his girlfriend's mom's bouquet or something.  My 21-year-old daughter called at 8 tonight and said "Happy Mother's Day. I didn't get you anything. I can get you a card though." I think she basically called to let me know she works tomorrow because I watch my 1-year-old grandson when she works. Oh yeah, my husband and I got in a fight. I'm so close to tears. I can't wait until everyone is bed so I can have a good cry. Am I being too sensitive about the older kids? I don't expect them to spend a bunch on me but they both work, and a $5.00 little flower, an annual or something, or pitching on a hanging basket or something, or even a card..whatever..would have made my day. I don't know. Maybe it's just PMS.
Mother's Day
I'm sorry about those 2 self-asorbed older kids. Just think how much the little one loves you - when a 5-year-old remembers Mother's Day, that's special!

As one of the other posters said, remember this on your kids' birthdays. Turnabout is fair play. If you keep giving to them and doing for them, they will get the message that they can treat you anyway they want.

Personally, I would call them on this, but that is just me. I wouldn't be watching that grandson, I can tell you that.

Oh gosh! Don't even get me started on mine --- that is another book! lol. Oddly enough the one thing she does approve of is me being here for her son and our children. She has opinions (her own-- not always bright) about everything else I don't do the way "she" would.

for her too long, however, it has been almost a year for me, maybe even longer with these palpitations etc. Some days I am great, others not so great. I was walking with a neighbor until she got pregnant but has had the baby now and I think exercise does help the whole situation.

She was the transcriber (that's what we were called back then) for a 10-man (yes, they were all men) multispecialty group. She needed help, so the summer I was 16, she dragged me in kicking and screaming and made me help her. I could type and spell and I had passed high school biology; what more was needed?

You can only write off the percentage actually used...
for work.  Who uses 40% of the space in their house???  I write off 10-15%.  You also must use that work area for work only.  Come audit time (and it eventually may happen), you will need to show a floorplan of your house and what area is for work exclusively.  If you're writing off 40% you will be in deep doo-doo.

You are exactly right. Ooooh let me buy that huge TV or put a brand new Lexus in the driveway. please. Christmas is about giving not about receiving and it is not about giving presents. It is our time and our love that is needed most, especially this time of year. We do the same here. Make presents for each other, make the decorations for the tree, donate our time and our money to those who need it. Soup kitchens, Salvation Army, local homeless shelters, etc, etc. We have never "not" been able to afford going crazy on Christmas presents for our own families, but really just never wanted. Our entire family feels it is much more important to spend on those who most need our help. Kudos for you and a very merry Christmas!!!!!!

I have done both and prefer IC because I can write
everything off. You have to be very diligent about receipts and setting aside tax money though.

The only real difference in pay between the two is 7.25% for social security (self employment. You have to put aside the taxes rather than have the company pay them for you but either way, that tax money comes out of your check not the company's pocket.
Mine was before I became an MT. I used to have to write

a lot, using a lot of fine motor grip, took maybe 2 years.  Typing doesn't bother me unless I do a 12 hour day, but then with rest it is usually better.


I suggest wearing a splint/splints to bed because you may bend your wrists in your sleep and that will contribute to the problem.  Keeping them straight really helps.  My orthopod recommended 15 minute soaks - cold, hot, and then cold again.


I recommend lowering your keyboard tray if possible and putting a pillow in your lap and then the keyboard on your pillow.  This helps take a lot of the stress off.  I also find that I sit straighter with my back pressed against the back of my chair.   If that doesn't work for you put a towel, folded at least once, under your keyboard on your tray and that will help take some of the stress off also, though not as much as the pillow.  If you use a normal-sized bed pillow there is enough pillow to also rest your wrists on and to me is just more comfortable than having an edge of  the wrist pad.  I couldn't get used to a gel one.   If you are using a mouse have it on your keyboard tray where you don't have to bend your wrist as much to use it.


There are vitamins/supplements that are supposed to help  too.  I'm sorry I can't remember when they are but you can Google it. 


My last surgery was 17 years ago and I find that when I grip things, like a rake when doing yard work that my wrists hurt, but it doesn't hurt when I type, different muscles I guess.   I have lost some strength in my hand.  I was the one that always opened the jars no one else could and now I'm not able to open them.

I've had so many funny things happen working in hospitals and doing medical transcription in hospitals and at home. I wish I had written them all down. We could all write a book! Today has been fun what with the polls and the trips down memory lane we've taken today.
I would write one more letter and CC: to
the insurance commissioner and your insurance company. Write the letter to your physician's office, enclose copies showing proof of your payment of co-pays one more time.  I found whenever I had CC to the Insurance Commissioner, then people got serious about finding the correct records...

Can I write off all my DSL charges
on taxes.  I assumed I could, but now somebody is telling me not if I use it for personal stuff that I would have to figure out how much I used it for personal, and how much I used it for work.  Of course I wouldn't have DSL if work did not require it, so why should I not be able to write off the whole cost. 
I write off everything I use for work- sm
part of the house electric -30%, heat 30%, phone (1 line is all work, and the other line I write off half as I am on the phone more for work than personal use), all my MT books, computer, printer supplies, programs, FTP services, etc. Whatever I use for work gets written off 100%. I keep receipts for everything and a log of the electric and propane costs.

I am sure I use it more like 3/4 time for work, but half to be safe. Tax Cut, from H&R Block, goes through the deductions with you and it is simple.

Quickly I want to say this - first, many of the rules that you are living by are self-imposed. You are extremely intelligent and eccentric and you are creating a mental cage for yourself, trapping yourself in your own mind by saying that you can't do this and can't do that. I have trapped myself in situations for years before realizing that it was within my own power to set myself free. It is for you too. Saying that you can't work for other people is inaccurate. It may be true that you do not want to work for others or do not enjoy it but it is not true that you cannot do it. Many people don't want to work for others but you may have to do it at least in the short-term while you wait for your online businesses to take off.

Also, as far as getting psychiatric help you can go to the emergency room. When you feel that you should never have been born, that is a sign that you need to go and get help. You need an antidepressant. At the very least you need some fresh air and sunshine. I will write more later.
You can write off your mileage

to get into.  As far as blocking your schedule, that's your fault, not the office's fault.  Would you charge an online company a "sit and wait" for dictation charge because you blocked your morning?  Hello?  You have to be very flexible in this business.  I have gone and there's only 1 tape to pick-up.  I really don't care to know the doctor's daily schedule because I have enough to worry about, but high-paying jobs are hard to find.  It sounds like you've made up your mind in that you don't like the driving, office, or the dictator himself, so what kind of advice are you looking for?  If you just want to know whether to charge for a pick-up that brings you home empty-handed, then no do not charge.  Why don't you talk to your overflow account and find out if this happens can you take work?  I just don't get some of you gals...  Really this allergist does not "owe you a tape" everytime you come in and I'm wondering if you are going to return reports.  If you are returning reports, then your trip was not wasted.   


P/S:  It could be worse; they could NOT PAY!!!!!  LOL  Good luck...

I am so tired of dealing with haughty people who don't have a clue


why I need something or what I do - and on top of them coping an


attitude with me because I get exasperated with their inability


to do their job which is make the work flow.


If MQ had people hired in their work flow areas who had worked inside


the world of transcription and knew where it goes in a hospital,


what to look at on the screens of the machines in the work flow rooms,


etc etc we wouldn't be getting hysterical messages all the time about


not meeting turn around times.


But no!  they want to hire first-time jobbers to be in control of the lives


of people who have been in the business usually upwards of 15 years


in order to even do this MT job at home with no help or assistance -


and then make us put up with them and at the same time try to make a living.


I don't want a doctor operating me who has never been inside a hospital, and I


am sick to death of dealing with people in a transcription company


who has never been inside a hospital and followed a REPORT around.


They need to see WHY A REPORT is done - HOW IT IS DONE


They need to go to dictate stations - they need to go in a medical records


area and look at charts - go inside an OR - and into the ICU


they need to see how the world of medicine is and how it operates.


Only then will they understand TATs, reports, and why things are done as they


are or at least why people want them done a certain way.


Like if I keep getting dictations with LOUD BUZZING - i don't want to keep getting


LOUD BUZZING - I want to let the hospital know there is probably a bad phone


instrument - and if this work flow person sees where dictation is done he will


understand it wouldn't take much to MAKE THE BUZZING STOP - and not get


pissy with me because I'd like to talk to somebody about LOUD BUZZING ON


REPORTS. - It's not rocket science - just need to let somebody know.

GLOSSARY OF MEDICAL TERMS
Areola The pigmented or darker colored area of skin surrounding the nipple of the breast.

Asymmetry A lack of proportion of shape, size and position on opposite sides of the body.

Autoimmune Disease A disease in which the body mounts an "attack," disease response to its own tissues or cell types. Normally, the body's immune mechanism is able to distinguish clearly between what is a normal substance and what is foreign. Breast Augmentation Enlargement of the breast by surgical implantation of a breast implant or patient's own tissue.
Breast Reconstruction Surgical restoration of natural breast contour and mass following mastectomy, trauma or injury.

Capsular Contracture Tightening of the tissue surrounding a breast implant which results in a firmer breast.

Capsulectomy Surgical removal of the entire capsule surrounding a breast implant.

Capsulotomy Closed Capsulotomy: Compression on the outside of the breast to break the capsule and relieve contracture.

Open Capsulotomy: Surgically cutting or removing part of the capsule through an incision.

Carcinoma Invasive malignant tumor.

Congenita Anomaly Abnormality existing at birth.

Connective Tissue Disease(CTD) A disease or group of diseases affecting connective tissue. The cause of these diseases is unknown. The diseases are grouped together on the basis of clinical signs, symptoms, and laboratory abnormalities.

Deflation/Rupture Refers to loss of saline from a saline-filled breast implant due to a tear or cut in the implant shell or possibly a valve leak.

Displacement Shifting in the original position.

Epidemiological Pertaining to the cause, distribution and control of disease in populations.

Extrusion A breast implant or tissue Expander being pressed out of the body.

Fibrous Tissue Tissue resembling fibers.

Hematoma A swelling or mass of blood (usually clotted) confined to an organ, tissue, or space and caused by a break in a blood vessel.

Immune Response The reaction of the body to substances that are foreign or are interpreted as being foreign.

Inframammary Below the breast.

Inframammary Fold The crease at the base of the breast and the chest wall.

Inframammary Incision A surgical incision at the inframammary fold

In-Patient Surgery Surgery performed in a hospital requiring an overnight stay

Latissimus Dorsi Two triangular muscles running from the spinal column to the shoulder.

Mammography Use of radiography (X-rays) of the breast to detect breast cancer. Recommended as a screening technique for early detection of breast cancer.

Mastectomy Surgical removal of the breast.

Subcutaneous Mastectomy: Removal of breast tissue, preserving the skin and nipple.

Partial Mastectomy: Removal of primary tumor and a wide margin of tissue, may include the overlying skin and the muscle fibrous tissue (fascia) underlying the tumor.

Total (Simple) Mastectomy: Removal of breast tissue and the nipple; sometimes accompanied by armpit (axillary) node dissection.

Modified Radical Mastectomy: Removal of breast tissue, nipple, and fascia of chest (pectoralis) muscle with axillary node dissection.

Mastopexy Plastic surgery to move sagging (ptotic) breasts into a more elevated position.

Necrosis Death of tissue. May be caused by insufficient blood supply, trauma, radiation, chemical agents or infectious disease.

Oncologist A specialist in the branch of medicine dealing with the study and treatment of tumors.

Out-Patient Surgery Surgery performed in a hospital or surgery center not requiring an overnight stay.

Mammaplasty Plastic surgery of the breast.
Mammary Pertaining to the breast.

Palpate/Palpability To feel with the hand.

Pectoralis The major muscle of the chest.

Plastic Surgery Surgery intended to improve, restore, repair, or reconstruct portions of the body following trauma, injury or illness.

Prosthesis An artificial device used to replace or represent a body part.

Ptosis Sagging of the breast usually due to normal aging, pregnancy or weight loss.

Rectus Abdominus Major abdominal (stomach) muscle.

Saline A solution of sodium chloride (salt) and water.

Seroma Localized collection of serum, the watery portion of blood, that resembles a tumor.

Serratus Muscle located beneath the chest's pectoralis major and minor muscles and the rib cage.

Silicone Elastomer A type of silicone that has elastic properties similar to rubber.

Subglandular Placement Placement of the breast implant behind the skin and mammary gland, but on top of the chest (pectoralis) muscle. Also called prepectoral or retromammary placement.

Submuscular Placement Placement of the breast implant under the chest (pectoralis) muscle, or under the pectoralis and serratus muscles. Also called retropectoral or subpectoral placement.

Surgical Incision Cut made in tissue for surgical purposes.

Transaxillary Incision Incision across the long axis of the armpit (axilla).

Umbilical Relating to the navel.

Unilateral Affecting only left or right breast.


Anyway, sorry, on a more serious note...
as regards your problem:  Do you have Ad-Aware and SpyBot and have you run those?  If you have run those and are still not finding anything, you might want to try a trial of this program I just downloaded myself and seems to have gotten rid of this darned WinFix (Virtumond?) pop-up problem I've been having recently that my Ad-Aware and Spybot couldn't seem to take care of.  Dang, I might actually buy this one!  But anyway, you can use it for 2 weeks, I think it is, for free (see link below).
Just a note: There are two MTs that I will not SM

use to this day - 15 years down the road - because they did this. They will never get a recommendation from me and they will never sub for me.

I know a lot of people believe as you do, but in my family I have seen lots of evidence to contradict this theory.

I don't condemn anyone who overdrinks, but I think we spend too much time in our culture blaming genetics and other people - mostly our parents - for our own poor choices and bad behaviors.

Bottom line is, the alcohol does not force itself into anyone's mouth and neither do the drugs. To me, drugs include not just the street drugs, but the legally obtained prescription drugs that so many people rely on to get them through the day (do not flame me about arthritis meds, etc. taken for legitimate conditions).

I wonder if any of you report errors you notice in other reports to the QA at your office. I have seen some doozies, but I admit I have been remiss. I just wanted to know what the rest of you do, even if the report is old.
