This site has all necessary forms and contact info for IRS, 800-829-1040. I suggest you also call them to ensure you are abiding by all rules. If you don't already, from my understanding, you should be paying taxes quarterly. I live and Alabama and it is the requirement here. Otherwise, I'm penalized.
