I have seen this happen in house and as a remote transcriber.  At my old job in-house, after we were on VR, one of the ladies was doing this and Information Services reported it. I have seen it happen and frankly I do not see anything that can be done unless IS backs it up.  People are so vindicative.  In many cases, others just can't believe someone is making the $$$ that they are for pure hard work.  Its a scary situation, but I seen it happen on numerous occasions.  I caught someone doing it to me inhouse.  I typed a four page interventional medicine report one day and printed off three extra copies and left one in the printer on purpose and put a sticker on the back of it.  I took another copy and put another color sticker on it and hid it under my CPU.  I took the other copy to the doctor for proofing because it had tables, and all kinds of extra stuff in it.  After I left for the day, Miss Smart-A** Hater pulled the marked copy and went it and changed things, and then linked the 5 other accessions incorrectly, all on my number/initials.  When I got to work the next day, boy was I in trouble for "gross negligence".  So I pulled my copy, pulled the doctors corrected copy and boy were there red faces and tears in that office.   It was a real mess, and I could have gotten the ax but I had the feeling she would do it.  She did it all the time and I was really tired of cleaning up her messes.  She finally left to go be a teacher.  (:  I guess backbiting is in every profession.   But I never figured out anything else to do to protect myself against that and I guess you really can't.  Just pray about it....
