First phalanx is okay, but 1st phalanx is better because it is recommended to use numerals in medical reports. (Book of Style)

Review of his x-rays today shows that he has some fourth and fifth proximal phalanx fractures of the shaft, just a little oblique in nature. The s/l 'ulnar-peri' was significantly displaced.
Hx: Fourth and fifth phalanx fractures.

In this op note, the doctor says a "one five recon plate" was trimmed to the appropriate length, and I placed a "one five" screw distally. Would you agree this should be transcribed as 1.5 (as in mm). He didn't say mm, and I won't go so far as to put mm in the report.
