An ASCUS pap smear result is considered to be mildly abnormal. This is the most common type of abnormal Pap smear. ASCUS stands for Atypical Cells of Undetermined Significance,and basically means there were mild cellular changes and the the cause in unknown.

 He is unfortunately far beyond our being able to do zank smears, IFAs, PCRs, or cultures off of the primary lesions.  Should we turn up a herpes type 2 lesions, we will at least broach the issue of hepatitis testing, in addition to HIV testing.

as part of the neurological exam, you have sensory testing and withdrawing to pain or (some kind of) stimuli would be correct. Not sure about the phraseology in your report, maybe 'withdrawal sensation' ??

On his hearing test, the patient could not hear frequencies at 1000-2000-4000 with his right ear at 40 decibels.  He could not hear 1000 with his left ear at 40 decibels.  However, during the whisper test, he was able to hear three items in each ear from at least 7 feet away.


 


I have no idea how to type the 1000 2000 4000.  This is on a physical exam.  Thanks for any help!!

CLINICAL IMPRESSION:

2. Bronchitis.  Rule out a right pneumonia.
3. Chronic lymphocytic leukemia.
4. Atrial fibrillation.  Rule out arrhythmia.



The reason I do it that way is because way before, we used to line Rule out under let's say Atrial fibrillation, yielding 2 lines.  It used to be done that way back in the early 90s.  Now it is not done that way.  So, I give the impression which would be atrial fibrillation and begin a new sentence....because they have to rule out an arrhythmia.  I don't think it is etched in stone, so either/or would be fine.  *S*

I have heard "estrogen receptor" and "progesterone receptor" positive in breast cancer, so think "estra septa" could be "estrogen receptor.

Not sure what "heptra" could be.

A plastic wound exposure fish was placed overlying the bowel to allow placement of the composite Gore-Tex and Prolene mesh patch and the mesh was secured. At the end, he said it again and it was clear that he was saying "wound fish", but for the life of me I can't find it said this way anywhere.
