    Sorry, should have been more specific. Dictator is probably slurring the word "ophthalmic" drops. There is no thalmic drop. The word "thalamic" refers to something else entirely. Please just type out ophthalmic drops.

    He probably does mean the injection will help clarify where the pain is coming from.  That is the whole purpose of a a discogram, but sometimes they are inconclusive.  It's just an awkward sentense structure to say clarify the patient.

    Doctor dictates Medications listed on s/l Card-ex from skilled nursing facility.    Then he goes on to list medications. 


    Some sort of form called "Card-ex"  is what I am looking for.

    Always go by your company policy and produces. My company states:

    "when the manufacturer uses all capitals, use an initial capital only so as not to give undue attention to the term in a medical report..."
    Also to help clarify, toco is

    Prefix meaning childbirth. For example, tocolysis is the slowing or halting of labor. Sometimes spelled tok-, toko-.

    So definitely not toca even though you hear it clearly, the docs are not always right.

    any procedure or disease attributed to a person's name should no longer carry an 's, thus Morton neuroma. I am really sorry to say this, as I really do appeciate QA support, but from what I have seen from your posts, you are very uninformed.
