The patient was taken to the operating area in supine position and had orotracheal intubation gas anesthesia delivered.  This was after the patient was prior marked in the holding area.  These were s/l wise pattern marks made so as to perform McKissock type breast reduction. 

Left breast has a significant capsular contracture, more so medially, where there seems to be some tethering of the pectoralis muscle which is displacing the implant laterally.

We may have to performan AlloDerm sling medially in order to reconstruct the ______________ of the breast.

mL refers to liquid measures, and cc (or cubic centimeters) to solid measures.   For some reason, I cannot get my brain to differentiate whether this 20 cc subcutaneous pacemaker pocket falls into the solid measure or liquid measure category.  I THINK it would be solid measure, but it's been a long day and my brain is totally zapped.  Appreciate any input!

an irregular area of enhancement  "demonstrating a suspicious _________ curve adjacent to the tract..."   Sounds like washout?  An area where a prior biopsy had been done. TYIA.

Examination at this time demonstrates diffuse fibronodularity in both breasts.

That's what it sounds like.  Any ideas on what the word could be?

I would type it this way or at least make it present tense.  On my accounts, I am required to categorize these things.

In the physical exam, s/l thenar and epithilial wasting.  He is listing liver disease stigmata and I just blanked out on this.  Help!

Physical exam, under Extremities, doctor states ___ pulses throughout.

It sounds like temporis pulses. I know of temporal pulses, but would that be throughout the extremities? Thank you!

EXAMINATION:  No costovertebral angle tenderness.  Abdomen has normal active bowel sounds in all quadrants.  It is supple.  Extremely tender left upper quadrant and going down not quite into the groin.  Difficult to check the (s/l sternal) border because of increased tenderness but the spleen may be mildly enlarged but no distinct masses felt.  I am unable to check for rebound or guarding because she is so exquisitely tender with even a very light touch.

ASSESSMENT:  Acute left upper quadrant pain.

s/l sternal border?  Any help would be great?

I am wondering if, in the last sentence, where "drop can arc" is dictated . . . would she be referring to three tests/maneuvers such as Drop Arm, Empty Can, and an Arc test, or is it correct as is?

She has excellent range of motion of the shoulder.  Good grip strength.  Distal pulses and sensation are intact.  She can do a full drop can arc.
