use arabic, some Roman. Grades are usually arabic.

Stage I breast cancer (T1B, sentinel N0, M0, ER positive, PR negative, grade 2, HER2/neu negative).

Lowercase stage and grade.

Use roman numerals for cancer stages. For subdivisions of cancer stages, add capital letters on the line and arabic suffixes, without internal spaces or hyphens.

stage 0 (indicates carcinoma in situ)
stage I, stage IA
stage II, stage II3
stage III
stage IV, stage IVB

Use arabic numerals for grades.
