Could you give a little more information?  Where there any other symptoms relative to the stool character?  Any other positive findings in GI tract (ROS, physical exam or lab)?  Diagnosis?  What was the exact content surrounding the patient's "consult"ing the Hemoccult card?  In a long stretch, an MD might "consult" the Hemoccult card by viewing it directly, but the patient is smearing it with stool and submitting it.

Subject: No they are three separate views.

No they are 3 separate views.
It is the sentence structure that is confusing. Three individual views, but once the word views gets put into the sentence it becomes plural and without the word views it is regarded as singular. I don't necessarily agree, but I believe this is why the QA department and your manager keep changing your grammar.

but it DOES depend on the client profile/preference like the other poster said.....I get this format all the time in REAL consult reports (not letters) and I just type *Dear Dr. Smith* - I have just seen your patient....blah blah (can usually go under reason for consult or HPI) and then do my headings as dictated.
