I'm doing a psychiatric consultation by an Asian dictator, and he says this patient has been living in s/l Fiocor House or ThreeCor House for the last several months due to alcohol and drug problems.  This is for one of my accounts in Michigan.  I can't understand this pronunciation well enough to get a good s/l for Google, so what would ya'll recommend doing to find this darned term?

Google on "degenerative disc disease" and "degenerative disk disease". I use "disc" when transcribing for my ortho clinic because, when I got the account and asked for samples, each and every sample on each and every report used "disc". No matter what your research tells you, you will do what the client says, especially when it doesn't really matter. And this does not really matter.

Subject: An 8.4- node is visible. The node measured 8.4 cm.

If the hyphen is used to modify, I am hoping that this is correct.  Please excuse my lack of ability to absorb this.  It seems like my entire report will be filled with hyphens as many nodes are seen...it just looks too busy to me to be acceptable and having a hyphen in one place to describe the same node should be the same as when the node is described at the end of a sentence...I should lobby for changes, huh?

8.4-cm node.

the node is 8.4 cm.

Please correct me if I am wrong.  Thanks.

Subject: uterus was palpable, s/l ring incised-sm

I don't know if he is saying something like ring incised or ring sized.

Past history says: She had four cesarean sections, laparoscopic tubal sterilization, and a laparotomy with left oophorectomy and abdominoplasty.

In pelvic exam: PELVIC: Skene, urethral, and Bartholin glands within normal limits. Vagina, good support. Cervix was closed. No lesions were seen. Uterus was palpable, ______. s/l "ring incised" or "ring sized."

Could he be saying "ring incised" meaning he feels the incision from previous surgeries. Could it be he is saying uterus is "ring sized?"
